This Band 6 post offers an exciting opportunity for a suitably qualified and experienced nurse to work within the Specialist Palliative Care Team in the Community North area of the County.
The post covers core hours of 09:00-17:00 and works across a seven day period, weekends and bank holidays are rostered across the team as per CDDFT policy. Core hours are 09:00-17:00.
Main duties of the jobThe successful candidate will provide a consistent high quality specialist palliative care service for patients and families. With significant post registration experience including recent experience in palliative care.
As a Palliative Care Nurse Specialist the post holder will:
- Assess, plan, implement and evaluate care for patients and carers.
- Provide specialist advice.
- Provides day to day supervision and/or clinical supervision to others.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Statutory RegistrationEssential- Registered Nurse (adult) on part 1 of the NMC register

QualificationsEssential- Professional knowledge acquired through degree or equivalent level of demonstrable professional knowledge/experience
- Supplemented by diploma level specialist training, experience, short courses
