Job summary
Following investment into the community hospitals wards we have an exciting opportunity to join our team. We are seeking x2 highly motivated and enthusiastic Band 6 Physiotherapist's to join our therapy rehabilitation teams based at in the North and South localities. Shotley Bridge Hospital 37.5 hour vacancy. Richardson Community Hospital 30 hour vacancy.

Applicants must be prepared to participate in a 6 day week service, covering bank holidays and Saturdays on a rota basis.

The successful applicant will be required to work across the North and South Community Hospitals in the Trust.

Main duties of the job
This post is for a specialist Physiotherapist full time.

The service will work over 6 days a week.

The successful applicant will independently manage a caseload in the community wards providing holistic, short term, goal led intervention following a comprehensive assessment. These patients will present with a wide range of complex problems and will allow you to develop a highly specialist knowledge

Promoting a Discharge to Assess model of care, patients will have their discharge facilitated by therapists at the most optimum point in their rehabilitation pathway.

About us

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Job responsibilities
  • To work as a member of the clinical team of physiotherapists, occupational therapists and therapy assistants within community therapy services.
  • The post holder will independently manage a caseload that includes some highly complex patients and provide specialist assessments to adults referred to the team who present with a range of physical dysfunction/or disabilities resulting from injury, illness, surgery or chronic disease, within a range of community settings and establishments (including Rehabilitation Wards/Units, Clients Homes, Care Homes and Prisons)
  • On an on-going basis the post holder will assess, diagnose, implement and evaluate individual programmes of care for patients.
  • The post holder will work within a multi-professional team (Consultants, GPs, nursing staff, Occupational Therapists, Social workers and other AHPs) regarding the management and discharge of patients under your care.
  • The post holder will deliver clinical education of all staff as required within community services

Special Requirements
Essential
  • Ability to travel and carry small items of equipment on a daily basis to fulfil the requirements of the post

Desirable
  • Ability to meet physical demands of the post
  • Commitment, motivation and ability to work flexibly within a multidisciplinary service

Experience
Essential
  • Comprehensive and relevant qualified experience in physical rehabilitation including recent general rehabilitation experience.

Desirable
  • Experienced in staff appraisal
  • Experienced in working in a variety of settings

Special Skills & Knowledge
Essential
  • In depth and up to date knowledge and skills in the assessment and treatment of a wide range of physical disabilities
  • Understanding of, and the ability to manage mental health and psychosocial issues relating to rehabilitation
  • Remain updated with professional practice and new research

Desirable
  • In depth and up to date knowledge and skills in the assessment and treatment of a wide range of physical disabilities
  • Understanding of, and the ability to manage mental health and psychosocial issues relating to rehabilitation
  • Remain updated with professional practice and new research

Qualifications
Essential
  • BSc Physiotherapy or equivalent
  • Evidence of CPD, including attendance at recent post-graduate courses relevant to the clinical fields in the specialism

Desirable
  • BSc Physiotherapy or equivalent
  • Evidence of CPD, including attendance at recent post-graduate courses relevant to the clinical fields in the specialism

Statutory Registration
Essential
  • HCPC registered physiotherapist

Desirable
  • Member of CSP
