Description

Band 5 Primary Care Mental Health Practitioner
Company: Coventry and Warwickshire Partnership NHS Trust
Job ID: C9444-26-0637

Job summary
Help shape the future of neighbourhood mental health care.

Coventry and Warwickshire Partnership NHS Trust (CWPT) is seeking enthusiastic Band 5 Primary Care Mental Health Practitioners to join our evolving neighbourhood health model. Embedded within Integrated Neighbourhood Teams (INTs), you will work alongside colleagues from primary care, GP practices, UHCW, Adult Social Care and VCSE partners to deliver joined-up, person-centred support within local communities.

This is an exciting opportunity to be part of a transformational approach to care, helping people access the right support at the right time, closer to home. As a key member of the INT, you will act as a bridge between mental health services and neighbourhood partners, ensuring mental health is embedded within multidisciplinary care and decision-making.

This is not a traditional community mental health role. You will work across organisational boundaries to improve integration, reduce fragmentation and support the development of innovative neighbourhood-based services. The role offers the opportunity to influence system-wide working, build strong partnerships and make a real difference to the lives of local people.

If you are passionate about partnership working, enjoy building relationships and are committed to improving outcomes for communities, we would love to hear from you.

Together, we can make mental health everyone's business and bring care closer to the communities we serve.

Main duties of the job
  • Work as an embedded member of an Integrated Neighbourhood Team (INT), alongside colleagues from General Practice, Primary Care, Adult Social Care, Community Services, UHCW and VCSE partners.
  • Provide mental health advice, guidance and support to multidisciplinary teams.
  • Undertake assessments and support early identification of mental health needs, helping to prevent deterioration and crisis.
  • Facilitate access to appropriate mental health, health, social care and community services.
  • Act as a key link between neighbourhood teams and specialist mental health services, supporting seamless care pathways.
  • Contribute to MDT meetings, complex case discussions and coordinated care planning.
  • Support individuals, carers and families to access local services and community resources that promote wellbeing and recovery.
  • Build effective partnerships across organisations to improve collaboration and reduce service fragmentation.
  • Identify and address barriers to accessing mental health support, particularly for vulnerable and underserved populations.
  • Promote prevention, early intervention and a population health approach, enabling people to receive care closer to home.
  • Contribute to service development and innovative neighbourhood-based models of care.
  • Support integration across health, social care and community services to improve outcomes for local people.
