Job Summary
The successful post holder will be required to expected to work within CYPS Mental Health Team, working in base and within the community. You will be reviewing and supporting service users and engage with members of the multidisciplinary team under the guidance of the Care Co-ordinators, Clinical Supervisor.

Provide responsive and clinical advice to patients, families, carers, staff, statutory agencies, third sector partner agencies and the public.

Holding a Caseload of young people offering direct intervention. Delivering evidenced based clinical interventions.

Consider carer needs and ensure carers and family are involved in care planning

The post holder will work as a member of the multi-disciplinary team ensuring that high quality individualised care is delivered.

Coordinate treatment planning, interventions, clinical reviews and discharge planning.

Provide clinical supervision to staff as delegated by the Clinical Leads.

Provide compassionate care that is based on empathy, kindness, respect and dignity.

Main Duties of the Job
To manage a caseload of young people under direction of Care Co-ordinator/Clinical Lead and provide ongoing assessment and recovery focused Care Planning for service users who do not have enhanced needs and where risks are assessed as low, under the supervision of the Pathway Clinical Lead.
To provide, under the direction of a Care Coordinator and Clinical Lead, effective treatment and intervention to service users with complex and substantial needs.
To deliver evidenced based clinical and psychologically based Interventions, with appropriate Training and supervision and as directed by Clinical Lead and Supervisor.
The post holder will work as a member of the multi-disciplinary team ensuring that high quality, individualised care is delivered and that this promotes Recovery and Wellbeing and maximises Independence.
To provide compassionate care that is based on empathy, kindness, respect and dignity.
To incorporate a client centred philosophy into nursing practice and client centred collaborative care.
Ability to work flexibly.
Demonstrate flexibility in the role. This may involve working across clinical pathways to support the needs of the service, whilst ensuring the delivery of high quality care at all times.

Person Specification
Education and Qualification
Essential
  • First Level Registration RMN/RNMH/RNLD Qualification
  • Experience of working with service users and carers in successful delivery of treatment plans
  • Complete preceptorship programme for newly qualified nurses
  • Multi-disciplinary team work.
  • Proven ability to develop therapeutic partnership with service users and carers.
  • Proven ability to be able to communicate interventions to service users.

Desirable
  • Further relevant post qualifying training
  • Previous experience in a community setting.
  • Experience of involving service users/ carers in evaluation/planning and monitoring of services

Knowledge and Experience
Essential
  • Knowledge of relevant contemporary professional theories of mental health care and evidence based interventions.
  • Understanding of the statutory issues which impact on service users.

Skills and Competencies
Essential
  • Ability to use IT system
  • Comprehensive continuous assessment of skills
  • Risk Assessment skills.
  • Risk Management and Safety Planning skills.
  • Ability to establish a rapport and demonstrate engagement skills with service users
  • Adaptability, flexibility, good communication skills and a commitment to multi-agency team work
