We have an exciting opportunity for a newly qualified or experienced Nurse to work as part of our friendly and dynamic multi-disciplinary care team. You must be an enthusiastic individual with excellent communication skills and motivated in your approach to care delivery to ensure we maintain a high-quality service for our patients.
You will be supported to manage a caseload and working as part of a larger multi-disciplinary multi agency team, liaising closely with our Primary Care Networks and Neighbourhood Teams as part of the out of hospital model of care.
The community nursing service is commissioned to provide services between 8am and 10pm, seven days a week, including evenings, weekends and bank holidays. Applicants will be required to work a shift pattern within these hours as part of the role although flexibility can be discussed. A driving licence and access to a car is essential.
Main duties of the jobAs a community RN, you'll provide care to patients in their homes, managing a range of complex health issues and collaborating with various other disciplines. You'll develop a broad range of nursing skills, including IV medication administration, palliative and end-of-life care, complex wound management, catheter and continence care, diabetes management, PICC line care, medication administration, and drain tube management. You'll need to make autonomous clinical decisions and create holistic care plans, with the support of your colleagues and Team Leaders. Comprehensive training and guidance will be provided by our two dedicated Clinical Lead nurses.

The Community Nurse will be able to work effectively as part of a team and will always act within the limits of their competence, knowing who to ask for help and support as required.
