As a Band 5 Community Nurse, you will join our valued Community Nursing Team based at Sheppey Community Hospital, providing care across Sheppey, Sittingbourne and the surrounding villages. Package Description
You will be joining a friendly and experienced community nursing team who are passionate about delivering outstanding care closer to home. Main Responsibilities
As a Community Nurse, you will provide high quality nursing care to patients within their own homes and community settings. You will assess, plan, implement and evaluate care, ensuring patients receive safe, effective and compassionate support throughout their healthcare journey. Working both independently and as part of a wider multidisciplinary team, you will build strong relationships with patients, families and carers, helping them manage their health and wellbeing while promoting independence wherever possible. You will have the opportunity to utilize and develop a broad range of clinical skills while managing a varied caseload. This role offers autonomy, variety and the chance to make a meaningful impact within your local community every day. Key responsibilities include:
  • Delivering safe, effective and person-centered nursing care in community settings
  • Assessing patient needs and contributing to care planning and review
  • Supporting patients with acute, chronic and complex health needs
  • Working collaboratively with GPs, therapists, social care teams and other healthcare professionals
  • Promoting patient independence, health and wellbeing
  • Maintaining accurate and timely clinical documentation
  • Contributing to service improvement and sharing best practice across the team

Person Specification
Essential
  • Registered Nurse with a valid NMC registration
  • Passion for delivering high quality, patient-centered care
  • Strong communication and interpersonal skills
  • Good organizational skills and the ability to manage a varied workload
  • Ability to work independently and as part of a multidisciplinary team
  • Flexible and adaptable approach to work
  • Confidence using IT systems, including Microsoft Office applications
  • Full UK driving license and access to a vehicle for work purposes

Desirable
  • Previous experience within a community, district nursing or primary care setting
  • Experience managing patients with long-term conditions or complex care needs
  • Understanding of community healthcare services and integrated care pathways
