Job IntroductionAs a Band 5 Community Staff Nurse, you will join our valued Swanley and Rural PCN Community Nursing Team in Dartford, North Kent. This is a rewarding opportunity to deliver care where it matters most, within patients homes and local communities. You will support patients with a wide range of health needs, helping them to remain independent and avoid unnecessary hospital admissions. This is a varied and autonomous role where you will build strong relationships with patients and carers while developing your clinical skills in a community setting. Whether you are newly qualified or have previous experience, you will be supported to grow and develop within a friendly and collaborative team environment.
