Job summary
Sunderland North Community Treatment Team are looking to recruit a caring and compassionate community nurse with excellent interpersonal and communication skills to work as part of our multi-disciplinary team offering treatment to clients experiencing moderate to severe mental health difficulties. Your responsibilities will include the management of clinics, administering and monitoring of medication and providing treatment for clients experiencing mental health difficulties, working closely with families and other professionals.

Team wellbeing and supporting each other is something which the Sunderland North Team are brilliant at. We understand that transitioning to a new role can be challenging therefore, each member of the team supports new staff members to develop their understanding of the Pathway.

Staff training and supporting professional development is encouraged in the team and there are a number of opportunities staff can access from both local and national resources. We also encourage staff to have a voice within the service and encourage feedback in relation to service development/improvement.

Main duties of the job
Your responsibilities will include the management of physical health clinics, administering and monitoring of medication and providing treatment for clients experiencing mental health difficulties, working closely with families and other professionals.

In collaboration with the Service User and Carer where appropriate, develop Recovery and Wellbeing focused Care Plans, ensuring regular reviews to monitor progress and adapt Care Plan as necessary.

Due to the complexity of the needs of our services users, there are often a number of professionals involved in their care and we very much take a team ownership of care planning and delivery. We are a multi disciplinary team consisting of Consultant Psychiatrists, Community Psychiatric Nurses, Psychologists, Occupational Therapists, Clinical Support Workers and Peer Supporters.

Working within an experienced and highly motivated clinical team you will be part of a service engaged in ensuring the best possible practice in contemporary mental health care implementing NICE guidelines. Your job role will be to provide treatment to service users, supporting families and carers, whilst working collaboratively with other professionals and third sector agencies. Collaboration with service users and their family's is at the centre of the service we provide.

Person Specification
Education and Qualification
Essential

• First Level Registration RMN/RNMH/RNLD Qualification
• Experience of working with service users and carers in successful delivery of treatment plans
• Complete preceptorship programme for newly qualified nurses
• Multi-disciplinary team work.
• Proven ability to develop therapeutic partnership with service users and carers.
• Proven ability to be able to communicate interventions to service users.

Desirable

• Further relevant post qualifying training
• Previous experience in a community setting.
• Experience of involving service users/ carers in evaluation/planning and monitoring of services

Knowledge and Experience
Essential

• Knowledge of relevant contemporary professional theories of mental health care and evidence based interventions.
• Understanding of the statutory issues which impact on service users.

Skills and Competencies
Essential

• Ability to use IT system
• Comprehensive continuous assessment of skills
• Risk Assessment skills.
• Risk Management and Safety Planning skills.
• Ability to establish a rapport and demonstrate engagement skills with service users
