Job summary
We are currently looking for a flexible and innovative Community Practitioner to join our experienced community neuro rehab multi-disciplinary team.

The post holder will be required to provide high quality Community Practitioner assessment and intervention skills to patients who have complex clinical needs as a result of Traumatic Brain Injury (TBI), to enable them to maximise their rehabilitation potential.

The successful candidate work in a range of settings including the patients own home and the local community, as well as providing support to clients who reside in residential and nursing care settings in Northumberland.

Service users may present with a combination of physical, cognitive, sensory, psychological, behavioural and social needs therefore a commitment to multi-disciplinary working is essential.

The successful candidate will have excellent opportunities for continuing professional development and be supported by work based learning and specialist training.

The successful candidate will work across the entire county of Northumberland and must be able to meet the mobility requirements of the post.

Main duties of the job

• To develop co-produced rehab plans with patients with TBI and refer on to internal and external specialists in accordance with the patients assessed need.
• Provide specialist clinical advice to Service Users/Carers, staff, statutory agencies, third sector partner agencies and the public.
• To regularly review rehab plans with patients and families and amend plans according to changing need.
• To consider Carer needs and ensure Carer and family are involved in Care Planning where appropriate.
• To deliver evidenced based clinical interventions.
• The post holder will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.
• Coordinate Treatment Planning & Interventions, Reviews and Discharge Planning.
• Provide clinical supervision to junior staff as delegated by Long Term Conditions Lead.
• To provide compassionate care that is based on empathy, kindness, respect and dignity.
• To incorporate a client centred philosophy into practice and client centred and collaborative care.

Job responsibilities
Our Community Practitioners are expected to undertake assessment, and implement evidence based rehab packages as part of an MDT. The Community Practitioner has a lead role in monitoring and co-ordinating rehab plans for complex cases involving multi-disciplinary working. They will be responsible for supervision of students and junior staff.

Person Specification
Education and Qualification
Essential

• Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
• Mentor Training.
• Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
• Proven track record of post registration clinical experience relevant to the post.
• Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
• Up-to-date knowledge/experience of relevant legislation.

Desirable

• Post Registration Qualifications in Psychological Interventions
• Experience of working within a community team.
• Non-Medical Prescribing

Knowledge and Experience
Essential

• 12 to 24 month band 5 qualified nurse experience dependent upon service need.
• Awareness and understanding of professional accountability, NMC standards and other local and national initiatives.

Desirable

• Understanding of difficult to manage/challenging behaviours.

Skills and Competencies
Essential

• Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs.
• Risk Assessment and Risk Management / Safety Planning skills.
• Demonstrate an understanding of effective & appropriate treatment options including a range of evidence based clinical approaches as identified in N.I.C.E. Guidance).
• Demonstrate effective written and verbal communication skills. Evidence of a high level of professional, organisational and leadership skills (self & others).
• Ability to adapt within a changing environment. Ability to work under pressure
• Ability to work effectively with other professionals using a multi-agency approach and Excellent interpersonal skills.
• Ability to work autonomously and within a team and Good conflict resolution skills.
