We are looking to recruit either 2 part time or a full time registered nurse to work within the Ashbourne Community Nursing service. We cover both Planned and Rapid work. Rapid shifts provide cover of the service until 18:30. We are a friendly, supportive team who work closely together to meet the needs of the housebound population within the Derbyshire Dales.

Main duties of the job
The successful applicant/s will be responsible for managing referrals into our services and providing evidence-based care to support patients in their own homes. To assess, plan, implement and evaluate individualised patient care promoting self care wherever possible in an efficient and clinically effective way, whilst maintaining privacy and dignity is vital.

Providing advice, guidance and support to junior staff in the delivery of high quality care and actively supporting the District Nurse/Senior Community Nurse in the management of the health care environment are integral to this role..

This great opportunity includes working alongside members of the MDT including Therapy Teams, General Practice and Specialist Services .

Education / Qualifications
Essential
  • RGN
  • Evidence of recent relevant training

Experience and Knowledge
Essential
  • Experience of working in the community
  • Evidence of research based/reflective practice

Desirable
  • Long Term Conditions experience
  • Wound management experience
  • End of Life Care experience
