Job summary
We are seeking a Trainee Advanced Clinical Practitioner (ACP) to join Primary Healthcare Derby - Derby City Team Up.

Ideally, you will have already completed your non-medical prescribing and clinical examination modules. Progression to Band 8a will be dependent on successfully completing an MSc in Advanced Practice and maintaining a comprehensive ePortfolio documenting your clinical development and competencies.

The service is managed by Primary Healthcare Derby, although you will be employed by Derbyshire Community Health Services (DCHS).

You will be based at Derby City Council House, with the potential requirement to work across the wider Derby City region. You will be expected to provide a high standard of person-centred clinical care to housebound patients, many of whom may be frail, elderly, and have complex healthcare needs.

You will receive support from a community GP and other experienced, qualified members of the multidisciplinary team, providing an excellent environment for developing your advanced clinical practice skills.

Due to the nature of the role, applicants must be able to travel independently across the locality and have access to a suitable vehicle for business purposes.

If you have a passion for life-long learning, enthusiasm for supporting the development needs of others, excellent communication skills and a commitment to continuous quality improvement we would love to hear from you!

Main duties of the job
The successful candidate will deliver first class clinical care to the frail and elderly people in Derby. This maybe within a residential or nursing home, or directly at a patients home as part of our home visiting service.

As an ACP you will:
  • Provide clinical leadership, including direct clinical care, and advice on complex issues
  • Establish a cohesive team approach for the ACP workforce development
  • Support the TeamUp service leads and managers with audit, innovation, and performance management
  • Lead on training needs discussions, based on audit findings, supervision outputs and individual staff knowledge gaps
  • Assist in feedback for staff appraisals.

You will exercise a high degree of autonomy through the application of advanced clinical skills in relation to the assessment, diagnosis, treatment and management of adults.

About us
Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.

It is an exciting time to join primary care and community services as the national Ageing Well programme gathers pace.

DCHS is working closely with the Primary Care Networks to transform the care of housebound and vulnerable people across Derbyshire through our Team-Up approach.
