Job summary
Derwent Practice is a friendly, patient-focused Practice seeking an enthusiastic and motivated Practice Nurse to join our dedicated clinical team. This is an excellent opportunity for a qualified nurse with a minimum of one years experience in practice nursing who is passionate about delivering high-quality, evidence-based care within a primary care setting.

Main duties of the job
We are seeking a skilled and compassionate Practice Nurse to deliver high-quality patient care within a busy primary care setting. The role includes undertaking diagnostic procedures such as venepuncture, ECGs, blood pressure monitoring and specimen collection, alongside managing chronic disease clinics and maintaining effective call and recall systems.

We are based in the food capital of Yorkshire, close to the beautiful Howardian Hills, York and the North York Moors. We have a dedicated and supportive team of healthcare professionals, including 10 GP Partners, Salaried GPs, Practice Nurses, Pharmacists, Physiotherapists, Advanced Nurse Practitioners and Mental Health Practitioners as well as a full complement of support staff. Job responsibilities
Clinical Responsibilities:
  • Diagnostic procedures: For example, venepuncture, blood pressure, height/weight, ECG and any specimens.
  • General health care and promotion of healthy lifestyles.
  • Manage patients with Chronic Disease, run specific clinics and maintain call/recall systems.
  • Chronic wound care as well as post-surgical wound care.
  • Administer injections under individualised prescription or PGD within training limits.
  • Maintain the cold chain of vaccines, including safe storage, rotation and disposal of vaccines and drugs.
  • Administer immunisations for example, influenza, pneumococcal, tetanus and childhood immunisations.
  • Participate in national screening work such as cervical cytology.
  • Provide directly and assist GPs with contraceptive services.
  • Assist GPs with minor surgery as required.
  • Lead on infection prevention and control.

Core Duties:
  • Work independently and as part of a busy Nursing Department, to meet the needs of patients and service delivery.
  • Be an active and integral member of the Nursing Team, attending team meetings and training sessions, supporting colleagues and working flexibly to cover annual leave or sickness as well as enhanced hours including Saturdays on a rota basis.
  • Act as chaperone if required/requested by patients and colleagues.
  • Work alongside HCAs and Student Nurses to develop their skills and offer mentorship, guidance and support to improve standards of care if needed.
  • Assist in maintaining a safe environment for the nursing team, patients and co-workers, ensuring that all statutory and local regulations (e.g. Health & Safety, COSHH, clinical waste management, infection control) are implemented and followed.
  • Ensure data is recorded accurately and timely onto patient records using SystmOne, with due regard for record management and patient confidentiality.
  • Ensure data is accurately recorded and coded onto relevant templates and documentation associated with patient health care.
  • Maintain and stock examination rooms and treatment rooms with agreed consumables and clinical equipment / instruments.
  • Collect data and participate in regular Practice audits and research projects.
  • Be aware of quality standards and targets as set out in NICE guidelines, NHS contracts, Quality and Outcomes Framework, etc., working to achieve these standards at all times.
  • Promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice.
  • Maintain annual registration with the NMC and other professional bodies (as required). Adhere to the Code of Professional Conduct.

Person Specification
Qualifications
Essential
  • Registered Nurse with current NMC registration
  • Evidence of continued professional development
  • Minimum of 1 year's experience working in general practice

Desirable
  • Post-registration qualification in Primary Care Nursing
  • Mentorship/Practice Assessor qualification

  • Experience in managing long-term conditions (e.g. diabetes, asthma, COPD, hypertension)
  • Experience in childhood and adult immunisations
  • Wound care and dressings
  • Phlebotomy
  • Knowledge of infection prevention and control procedures
  • Understanding of safeguarding children and vulnerable adults
  • At least 1 year of experience in a general practice setting
  • Experience of working autonomously within professional scope of practice
  • Experience of working as part of a multidisciplinary team

Desirable
  • Competence in cervical cytology
  • Experience with chronic disease review templates and QOF
  • Experience using primary care clinical systems (SystmOne)
  • Participation in clinical audit or quality improvement initiatives
