Job summary
This position is for a part time role.

We are seeking a motivated and compassionate Registered Nurse who will support the delivery of community-based prevention, early intervention and physical health programmes across GP practices and community settings.

The postholder will deliver evidence-based nursing care, health promotion and preventative interventions as part of the Federation's Prevention and Physical Health Programme, including:
  • Atrial Fibrillation (AF) case finding and stroke prevention
  • Severe Mental Illness (SMI) annual physical health checks
  • Cardiovascular disease prevention
  • Population health management initiatives
  • Health promotion and Making Every Contact Count (MECC)
  • NHS Health Checks (where commissioned)
  • Other prevention programmes commissioned by the Federation

This is an excellent opportunity for someone with experience in healthcare or community settings who is passionate about prevention, patient care, and improving population health outcomes. The successful candidate will be approachable, organised, and able to work both independently and as part of a wider team whilst maintaining professional boundaries and high standards of confidentiality and patient care.

Main duties of the job

1. Prevention and Population Health
Support delivery of prevention programmes by:
  • Identifying eligible patients using agreed inclusion criteria
  • Delivering prevention clinics within practices and community settings
  • Supporting population health initiatives
  • Promoting self-management
  • Reducing health inequalities
  • Supporting early identification of long-term conditions

2. Physical Health Assessments
Undertake comprehensive nursing assessments including:
AF Case Finding
  • Single-lead ECG recordings
  • Pulse rhythm assessment
  • Blood pressure monitoring
  • Cardiovascular risk assessment
  • Recognition of abnormal findings requiring GP review

SMI Physical Health Checks
Undertake comprehensive annual physical health reviews including:
  • Blood pressure
  • Pulse
  • Oxygen saturation
  • Temperature
  • Height
  • Weight
  • BMI
  • Waist circumference (where appropriate)
  • Venepuncture
  • Lifestyle assessment
  • Smoking assessment
  • Alcohol screening
  • Physical activity assessment
  • Cardiovascular risk assessment
  • Diabetes risk assessment
  • Physical health questionnaires

DGS Health is a GP Federation based in Gravesend, North Kent. We are comprised of 100% of local GP practices across Dartford, Gravesham and Swanley. We pride ourselves on being clinically led and managerially enabled and like to ensure we recruit people who share our ethos and culture. We are a small and friendly but growing organisation, employing over 33 people across a range of roles. Our services include urgent treatment services, paramedic home visiting, care home support and frailty nursing. 3. Clinical Decision Making
Working within competence, the nurse will:
  • Recognise abnormal clinical findings
  • Assess patients holistically
  • Prioritise clinical need
  • Escalate concerns appropriately
  • Refer to GPs or other professionals
  • Recognise deteriorating patients
  • Initiate emergency procedures where necessary

The postholder will not diagnose medical conditions independently but will exercise professional nursing judgement within their scope of practice.

4. Health Promotion
Deliver evidence-based health promotion including:
  • Smoking cessation advice
  • Healthy eating
  • Weight management
  • Physical activity
  • Alcohol reduction
  • Cardiovascular disease prevention
  • Diabetes prevention
  • Mental wellbeing
  • Vaccination awareness
  • Cancer screening awareness

Apply Making Every Contact Count (MECC) principles during all patient contacts.

5. Patient Education
Provide patients with information regarding:
  • AF and stroke prevention
  • SMI physical health
  • Long-term condition prevention
  • Lifestyle modification
  • Medication adherence (within competence)
  • Accessing local services
  • Self-management strategies

6. Clinical Documentation
Maintain accurate contemporaneous clinical records by:
  • Recording assessments in EMIS
  • Completing clinical templates
  • Coding activity accurately
  • Recording referrals
  • Maintaining data quality
  • Ensuring records comply with NMC standards

7. Communication
Communicate effectively with:
  • Patients
  • Families
  • Carers
  • GP Practices
  • Community Teams
  • Secondary Care
  • Public Health Teams

Escalate concerns promptly.

8. Safeguarding
The postholder will:
  • Recognise safeguarding concerns
  • Assess risk
  • Follow safeguarding procedures
  • Make referrals where appropriate
  • Document safeguarding concerns accurately

9. Infection Prevention and Control
The postholder will:
  • Comply with IPC policies
  • Maintain equipment
  • Clean clinical equipment
  • Use PPE appropriately
  • Report IPC risks

10. Quality Improvement
Participate in:
  • Clinical audit
  • Service evaluation
  • Incident reporting
  • Learning from incidents
  • Clinical governance
  • Continuous quality improvement

11. Professional Responsibilities
The Registered Nurse will:
  • Maintain NMC registration
  • Work within the NMC Code
  • Maintain clinical competence
  • Participate in appraisal
  • Participate in clinical supervision
  • Complete mandatory training
  • Maintain a professional portfolio
  • Support students and junior staff where appropriate

Qualifications

  • Registered Nurse Adult Level 1
  • Current NMC Registration
