Job summaryThe Community Mental health Team (CMHT) for older people is based at Weymouth Community Hospital. The team offer assessment, support, treatment, and advice to older people (over 65) who have moderate to severe mental health conditions. We offer a range of psychological interventions, including Art Therapy. We also support people with a dementia diagnosis who present with a degree of risk or behavioural and psychological symptoms of dementia (BPSD).
The team is made up of registered mental health nurses, Psychologists and Consultant psychiatrist. The multi-disciplinary team offer specialist interventions which support people in managing their mental health and wellbeing. The team link extensively with a wide range of partners from the Health Service, including the local frailty team, Dorset County Council and Local Community services in enabling individual health and social care needs to be met.
We are fortunate enough to be participating of lots of local developments including the development of the integrated neighbourhood team and Access Wellbeing. This is an exciting time to come and join the team, where we are in a newly refurbished environment.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.Main duties of the jobThe post holder will work as part of an established integrated multi-disciplinary team, this team is made up of a Psychiatrist, Nurses, other mental health practitioners and Support Time Recovery Worker with further support from the business support team and psychologist.
We are looking for someone who is reliable, committed, dynamic, and a motivated professional with excellent communication, organisational skills and the ability to prioritise work.
Having experience of working in a community mental health environment would be useful, but not essential, you will need to be able to apply the philosophies of engagement and recovery within your work with service users.
A good level of IT literacy is an essential requirement.
The team is based in Weymouth and and covers Weymouth & Portland. The post holder will need to have a valid driving licence and use of a car as they will be required to assess and monitor patients in their own homes and within other community settings.

Person SpecificationRole SpecificEssential- Applicant must be a car driver and have access to a vehicle
Knowledge, Skills and TrainingEssential- Registered mental health nurse to degree/diploma level.
- Membership of the relevant professional body
Desirable- Learning and assessing in practice qualification or equivalent practice assessors training
Job specific experienceEssential- Experience at RMN Band 5 level.
- Demonstrable knowledge of assessment and therapeutic interventions in comparable role.
- Ability to prioritise and organise workload effectively.
Desirable- Recent previous experience within a comparable role.
Information technology/resourcesEssential- Experience of using electronic patient/service user record systems
Desirable- Able to analyse data and produce reports using Microsoft Excel and Word.
