An exciting secondment opportunity has arisen for an experienced and enthusiastic Community Nurse to join the Bollington, Disley and Poynton Team as a Band 6 Community Caseload Manager.

Our team is based at Priorslegh Medical Centre in Poynton and provides care across the Bollington, Disley and Poynton area, supporting a local population of approximately 33,000 residents. We are a committed, friendly and forward-thinking multidisciplinary team with a shared vision to deliver the right care, at the right time, for the first time, helping to improve the lives of our local residents.

We are looking for a motivated nurse with a positive and proactive approach who can provide clinical leadership, support effective caseload management, and work collaboratively with colleagues across community nursing, therapy services and primary care. The successful candidate will work closely with the therapy team and GPs from the Middlewood Partnership to support safe, coordinated and person-centred care.

The post holder will help support the delivery of the NHS Long Term Plan and the ongoing development of integrated neighbourhood working.

Main duties of the job
The Community Caseload Manager will provide holistic, high-quality and safe care to patients, carers and families within their own homes and community settings. The post holder will undertake comprehensive assessments for new and existing patients, including those with complex needs and multiple conditions, using specialist clinical reasoning to plan, implement and evaluate individualised care.

They will manage their own caseload and provide proactive oversight of the wider team caseload, ensuring referrals, discharges, handovers and care planning are coordinated safely and effectively. The role includes working closely with the multidisciplinary team, GPs, social care, voluntary agencies, independent providers and families to support integrated care, avoid unnecessary hospital admissions, and facilitate safe transfers from hospital.

Qualifications
Essential

• Registered Nurse
• Specialist Nursing Practice (District Nursing) Degree or equivalent experience (and be willing to undertake/complete training within 2 years of commencing in post)
• Community Nurse Prescribing (V150)
• Mentor/Preceptor/998 or equivalent

Desirable

• Management/leadership Qualification
• Working towards MSc (Master of Science) in relevant subject
• Post registration qualifications e.g. palliative care, care of the elderly, wound care, continence or other relevant qualification
• Clinical examination Skills

Knowledge and Skills

• Demonstrable experience at band 5 with either community experience or transferable skills
• Evidence of clinical knowledge relevant to the post, with knowledge of care planning
