Job summaryWe are developing an Integrated Community Stroke Service (ICSS) within Great Yarmouth & Waveney, bringing together community stroke pathways into a more coordinated, responsive and patient-centred model of care. The role will involve inputting data sets on a regular basis to the sentinel stroke national audit programme (SSNAP).
This audit is a clinical and organisational audit to improve stroke care in the UK. The clinical audit is patient-level care tracking metrics for the integrated community stroke rehabilitation service and the organisational audit looks at structural and systemic resources to supply stroke services.

Main duties of the jobAn exciting opportunity has arisen within East Coast Community Healthcare (ECCH) to expand the participation and representation of ECCH services within the Sentinel Stroke National Audit Programme (SSNAP) with the development of the integrated community stroke service.
To provide professional, comprehensive and efficient management of collection and entering of SSNAP data.
To input data sets into the SSNAP database, ensure accurate recording of patient details relevant to the SSNAP domains.

Job responsibilities- Manages independent workload within set boundaries and with day-to-day oversight.
- SSNAP data administrator works closely with Multidisciplinary clinical stroke teams to manage and secure patient care metrics.
- Role will include data extraction and entry from clinical records and service level dashboards.
- Deadline management - ensuring all required clinical records are locked and approved prior to strict, quarterly national data locking deadlines. Ensuring data quality and accuracy.
- Cross team coordination - Managing patient record transfers between acute hospital stroke units and community rehabilitation teams to prevent duplicate tracking.
- Handles complex queries, confidential correspondence and sensitive data.
- Maintains Specialised databases of the Sentinel stroke National Audit programme (SSNAP) and service specific dashboards to meet key service indicators.
- To raise Quests for any issues relating to KPI breaches, system / process failure, etc.
- Assist with regular reviews of processes and procedures.
- Attend regular meetings with service leads or team leads as required.
- Attend regular team meetings, contributing in a positive manner.
- Ensure mandatory training is completed.
- Any other duties, which may be required to ensure the efficient running of the service.

QualificationsEssential- NVQ Level 3 or equivalent (or working towards)
Desirable- Formal Training in complaints / conflict management or equivalent
ExperienceEssential- Previous experience of dealing with patients in the NHS
- Previous experience of dealing with complaints
- Experience of conflict management
Desirable- Data extraction and entry from clinical records and service level dashboards
- Previous experience of statistical analysis
- Mixture of secondary care / primary care experience in employment history
