Job summary
We are offering an exciting opportunity for a Band 5 NMC Registered Nurse to join our Out of Hours Community Nursing team, with vacancies across Stoke-on-Trent, Stafford and Norton Canes.

Our service operates overnight between 16:00 and 08:15, seven days a week, providing urgent nursing care to patients in their own homes. We cover approximately 1,047 square miles, so successful candidates must be willing and able to travel throughout the locality. A valid driving licence and access to a vehicle are essential.

This is a varied and rewarding role, delivering high-quality, patient-centred care. With increasingly complex healthcare needs managed in the community, you will use and develop a wide range of clinical skills while making a meaningful difference to patients and their families.

Main duties of the job
As a Band 5 Registered Nurse, you will deliver high-quality care within patients' homes, residential settings and, where appropriate, clinics. You will assess health and care needs and contribute to the planning, delivery and evaluation of individualised care that promotes independence and positive outcomes.

This varied and rewarding role requires strong clinical skills, effective decision-making and autonomous working. You will:

• Prioritise care according to clinical need and health status.
• Undertake holistic assessments and develop care plans.
• Deliver palliative and end of life care, including symptom management and family support.
• Undertake catheter management and related procedures.
• Assess and treat acute and chronic wounds.
• Support long-term condition management.
• Deliver interventions to prevent avoidable hospital admissions.
• Complete Tissue Viability Risk Assessments and preventative measures.
• Work collaboratively with multidisciplinary teams and partner agencies.
• Support and supervise colleagues, including workload allocation and mentorship.
• Contribute to service improvement and development.
• Travel independently across a large geographical area, involving significant driving.
• Work flexibly across seven days, including evenings, nights, weekends and bank holidays.

You will develop a broad range of community nursing skills, with extensive support, training and development opportunities to help you grow and progress within the service.

Person spec
Essential

• Registered Nurse with current NMC registration
• Evidence of ongoing professional development
• Experience of undertaking holistic patient assessments and developing care plans.
• Understanding of palliative and end of life care principles.
• Full UK driving licence with access to a vehicle for work purposes
• Ability to travel independently across a large geographical area.
• Willingness to work evenings, nights, weekends and bank holidays as part of a seven-day service.

Desirable

• Community Nursing qualification or willingness to undertake further community nursing training.
• Mentorship, assessor or supervisor qualification.
• Ability to embrace change and support service development.
• Competence in palliative and end of life symptom management.
• Experience using RiO or other electronic patient record systems.

Job requirements
Essential

• Current NMC registered general nurse (degree in nursing)
• UK Car and driving licence
• Relevant experience
• Evidence of clinical knowledge relevant to the post, with knowledge of care planning. Understanding of Information Governance, Data Protection, NMC standards and Safeguarding Vulnerable Adults
• Flexibility to work across 7 days
• Basic IT skills
