Job summary
Applications are invited from enthusiastic, motivated, and committed individuals to join our friendly and dynamic team within the urgent community response team, REACT (reactive emergency assessment community team) and Hospital at Home.

REACT works closely with the department of older people's services at Ipswich Hospital and there will educational opportunities available linked to the OPS department.

This role is shared with other trust grade doctors. It provides excellent opportunity to gain further experience in acute medicine and rehabilitation/community geriatrics and would provide a good grounding for someone interested in either GP or geriatric training.

Main duties of the job
The post holder will be responsible for providing the medical review of patients within their own home environment, who are on the REACT and Hospital at Home caseload. This will include the planning and management of complete episodes of care, initiation and management of therapeutic diagnostics and interventions, working in partnership with the multidisciplinary team, and delegating and referring as appropriate to optimise health outcomes and resource use. The REACT caseload is discussed on the daily morning handover and the Hospital at Home has a daily MDT. Senior medical cover is provided by Dr Coutts, Dr Coates, Dr Gummer (GPwER) and the clinical nurse lead for REACT. Consultant level support is available Monday-Friday 9-5pm.

Person Specification
Essential

• Full registration with GMC
• MBBS or equivalent
• MRCP part 1
• Some experience at CMT /VTS level of acute geriatric and general medicine
• Some NHS experience
• Evidence of working in a multidisciplinary team
• Effective communication skills
• Flexibility to respond to changing service needs
• Valid driving licence and ability to drive in the community
• Commitment to good team working and relationships
• Commitment to training and continuing medical education

Desirable

• Full MRCP
• 6 months of geriatric training
• IT skills
