Key Responsibilities
  • To provide triage, assessment, diagnosis, and appropriate treatment/referral to patients according to their needs, either in the Practice or patients homes.
  • To support the development of an on the day emergency patient service within the Practice.
  • To undertake and interpret diagnostics and take appropriate action to manage patient care, distinguishing between abnormal and normal findings in order to relate them to a possible diagnosis, using clinical guidelines as appropriate.
  • To signpost appropriately to other services in the identified healthcare pathway.
  • To refer patients to appropriate care settings, within locally agreed healthcare pathways.
  • To discharge or treat, as appropriate and within the practitioners scope of practice.
  • To maintain accurate and systematic documentation of patient interactions and clinical care provided, including adverse events.
  • Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care

Main duties of the job
  • To maintain accurate and systematic documentation of patient interactions and clinical care provided, including any adverse events, child protection issues or other notifiable matters. Ensuring that documentation is of a high standard reflective of Advanced Clinical Practice, adhering to local and national guidelines.
  • To liaise and interact with multi-professional teams across organisational boundaries, including primary care, social services and secondary care providers according to patient needs and appropriate care pathways.
  • Promote collaborative working relationships and effective communication between all members of the team.
  • To maintain communication with all healthcare professionals and outside agencies to ensure seamless, patient-centred service provision.
  • Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
  • To assist in maintaining accurate data collection.

  • To ensure practice is within good clinical governance and based on sound up-to- date clinical evidence. Where appropriate, to identify and assess any risks associated with patient care or to oneself and communicate these risks to the appropriate clinical manager.
  • Be aware of and adhere to all relevant guidance and legislation relating to: Data Protection, Patient Confidentiality, GDPR, and Data Security

Learning & Development
  • Maintain own competences.
  • Disseminate learning and information gained to other team members in order to share good practice and inform others about current and future developments (e.g. courses and conferences). Assess own learning needs and undertake learning as appropriate.
  • To review and expand the role over time in line with the practices needs and the practitioners development goals

Job responsibilities

Delivering a Quality Service
  • To operate as an effective member of the Practice team.
  • Recognise and work within own competence and professional code of conduct as regulated by the Health and Care Professionals Council.
  • Produce accurate, contemporaneous records of patient consultation, consistent with legislation, policies and procedures.
  • Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
  • Deliver care according to NSF, NICE guidelines and evidence-based care.
  • Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.
  • Initiate and participate in the maintenance of quality governance systems and processes, including clinical audit, across the organisation and its activities.
  • Within the practice, and in partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate.
  • Support and participate in shared learning across the practice and wider organisation.
  • Understand and apply legal issues that support the identification of vulnerable and abused children and adults, and be aware of local statutory child/vulnerable patients health procedures and local guidance

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
