Job summaryFICS are looking for an Advanced Paramedic Practitioner to work as part of a multi-disciplinary team of AHPs/GPs/HCAs etc, providing care for the presenting patients within our Urgent Same Day Care Service at FICS - an established integrated service supporting the GP Practices of Farnham. This role requires 4 to 7 sessions per week between the hours of 9.00am to 6:30pm Monday to Friday.
Main duties of the jobThe Advanced Paramedic Practitioner role at Farnham Integrated Care Services (FICS), Farnham Centre for Health, will be a registered Paramedic with post graduate qualifications, including the Independent and Supplementary Prescriber qualification and HCPC registration, acting within their professional boundaries. From initial history taking, clinical assessment, diagnosis, treatment and evaluation, they will demonstrate safe, clinical decision-making and expert care for patients working collaboratively within a multi-disciplinary team.

Job responsibilitiesThe following are the core responsibilities of the Advanced Paramedic Practitioner
- Alongside a multidisciplinary team, to provide a first point of contact for patients of all ages presenting with undifferentiated, undiagnosed problems. You will provide an efficient same-day care service to our patients with a caring and respectful disposition. Working independently face to face or via telephone consultations, you will be accountable for delivering safe, clinical decision-making and expert care to a high standard.
- Acting within your professional boundaries, your duties will include all (or part) of the patient journey including initial history taking; clinical assessment; diagnosis; treatment; prescribing and evaluation of their care. Making use of your skills in history taking, physical examination, problem-solving and clinical decision-making to establish a diagnosis and management plan
- You will also provide enhanced care and management to frail and housebound patients, delivering long-term condition support, holistic care, and improving patient outcomes in alignment with Quality and Outcomes Framework (QOF) targets. This includes proactive, high-quality care management to meet both individual patient needs and PCN-wide QOF indicators.
- To refer patients to an alternative care setting or treat and discharge as appropriate
- To instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports
- Generate prescriptions within scope of practice, considering local and national guidelines.
- Deliver specialist care in line with required qualifications, including advanced interventions and clinical decision-making for complex cases
- Use prescriptive authority (where qualified) to enhance medication management, especially in managing long-term conditions, with attention to QOF outcomes
- Provide patient and family education on managing health conditions, preventive care strategies, and self-management aligned with QOF indicators.
- Candidates with experience or relevant job titles of Enhanced Nurse Practitioner, Advanced Paramedic Practitioner or Advanced Nurse Practitioner may be considered for this role.
Person SpecificationExperienceEssential- Evidence of experience working in General Practice, Urgent Treatment Centre, Out of Hours, Emergency Department, Minor Injuries Unit or equivalent autonomous clinical setting.
- Advanced clinical assessment skills
- Awareness of telephone triage assessment
- Management of common minor injuries/illnesses and knowledge of needs of patients with long-term (chronic) conditions
- Good clinical system IT knowledge including EMIS
QualificationsEssential- Masters degree level in the relevant area of expertise or is working at masters level or equivalent that encompasses all four pillars of clinical practice - clinical practice, leadership and management, education and research.
