Job summary
The Wellbeing Hub Co-ordinator plays an important role within the wellbeing service to help collate referrals, make initial contact, and ensure participants are triaged to the correct service. They will work closely with GPs and primary care professionals to identify and manage a caseload

The Wellbeing hub consists of;

Social Prescribers who empower people to manage their health and wellbeing via referral to non-medical link workers who focus on what matters to me, connecting people to community groups and statutory services.

Health and Wellbeing Coaches who support people to take pro-active steps to improve the way they manage their physical and mental health conditions, based on what matters to them.

The Pain Cafe, which is a seven-week course designed to empower people living with persistent pain to better understand and manage their condition. Through a combination of education, practical strategies, peer support and goal setting, participants can build confidence, improve their wellbeing and develop techniques to help them live well despite ongoing pain. The course focuses on self-management, helping individuals take control of their symptoms and improve their quality of life.

Main duties of the job
The Wellbeing Hub Coordinator is focused on coordinating referrals, supporting patients to access the right wellbeing services, managing communication between professionals and patients, promoting personalised care, supporting the delivery of the Pain Cafe and helping to deliver the Wellbeing Hub service across the PCN.

About us
First Coastal PCN covers a population of approximately 84,000 patients this is made up of seven practices, Marsico Medical Practice (Mablethorpe), Hawthorn Medical Practice (Skegness), Beacon Medical Practice (Skegness), Merton Lodge (Alford), Spilsby Surgery, Stickney Surgery and Old Leake Medical Practice. These practices work together to deliver care to the local population.

Job responsibilities
  • Help collate referrals, make initial contact, and triage participants to the correct service, complete triage calls for all services within Wellbeing Hub.
  • Work closely with GPs and primary care professionals to identify and manage a caseload.
  • Demonstrate flexibility/adaptability in a dynamic, evolving service.
  • Develop trusting relationships with participants; take a holistic, person-centred approach.
  • Support development of Personalised Care and Support Plans, liaising with practice teams, patients/carers, and the Neighbourhood Team.
  • Proactively outreach to participants on a regular, agreed basis.
  • Use knowledge of local services for signposting/referrals.
  • Support PCN targets for numbers seen by a Social Prescribing Link Worker and Health and Wellbeing Coaches
  • Scope and map local services (welfare, employment, healthy lifestyle, social, support); maintain databases of services/activities.
  • Engage other services to raise awareness of The Wellbeing Hub.
  • Manage effective use of resources/services for consistent quality and quantity of care.
  • Adopt multi-disciplinary, multi-agency approach.
  • Know when to refer to other health professionals/agencies.
  • Accurately maintain participant records.
  • Manage waiting lists (if applicable).
  • Understand barriers to accessing community support and use insight to shape support.
  • Promote the service within the PCN (to users and clinicians); build positive working relationships.
  • Reflect on and evaluate the service for continual improvement.
  • Communicate effectively with colleagues, patients, and carers.
  • Coordinate/navigate care for people and carers across health and care services, working closely with link workers, health and wellbeing coaches, and other primary care roles.
  • High level of written/verbal communication skills.
  • Highly motivated, proactive, strong organisational/time management skills, "can do" attitude.
  • Support with smooth running, organisation, and facilitation of The Pain Cafe.
  • Hold regular triage meetings with the Mental Health social prescribing team
  • Ability to travel across the local area when required.

Participation in the PCN mandatory training programme

Taking responsibility for own development, learning and performance, demonstrating skills and activities to others who are undertaking similar work.

Willing to develop advance skills to Masters Level

Person Specification
Experience
Essential
  • At least 2 years' experience of working in health, social care or other support roles in direct contact with people, families or carers (in a paid or voluntary capacity).
  • Experience of working within multi-professional team environments.
  • Experience of working with elderly or vulnerable people, complying with best practice and relevant legislation.
  • Ability to collect and record information and data, for record-keeping, monitoring and evaluation.

Desirable
  • Experience or training in person-centred care planning

Qualifications
Essential
  • Good standard of general education.
  • Grade C GCSE English and Maths or equivalent qualifications.
  • Excellent computer skills including email, word and excel
  • Full UK driving licence and use of car insured for work purposes

Special Skills/Aptitudes
Essential
  • Strong organisational skills including planning, prioritising, time management, report writing and record keeping
  • Ability to recognise gaps in services and to identify and act on safety concerns
  • Ability to recognise and work within limits of competence and seek advice when needed
  • Evidence of good verbal and written communication skills
  • Ability to build and maintain long-term-working relationships with colleagues
