Job summary
An exciting opportunity has arisen for a dynamic staff nurse to join First Community Health and Care working with the Responsive Services Team.

Responsive Services consists of the Urgent Community Response Team, Intermediate Care Team, Virtual Ward and Referral Hub.

This role with work in in collaboration with Primary Care and Secondary Care Colleagues, Urgent Community Response, Intermediate Care (Responsive Services), Community Nursing and other Specialist Health Care Teams across First Community, statutory and voluntary agencies to deliver excellent standards of patient care for patients who are acutely unwell requiring support to avoid admission to the acute hospital. Providing practical care, education and training for patients and carers admitted to responsive services.

Main duties of the job
You will complete assessments in the patients home and undertake treatments including IV medications, wound assessment, and management. You will work closely with the multi-disciplinary team to promote independence of patients to ensure they can remain in their own home.

The candidate will be able to adapt to change and work in a busy environment. Excellent communication skills are essential to enable liaison with all our health and social care partners in the management and assessment in the community.

Due to the nature of the role it is essential that applicants are able to drive, have a car suitable for business purposes and have a clean driving licence.

FCH&C is committed to safeguarding and promoting the welfare of vulnerable adults and children and expect all staff to share this commitment. FCH&C will support personal development through the annual appraisal process and in house training.

The service runs from 8am-8pm 7 days a week, 365 days a year, the post holder will be expected to work on a rota basis. There are opportunities for both full time and part time hours.

Job responsibilities
The post holder will work under the direction of the Responsive Services Charge Nurse/Sister as part of the Responsive Services team which consists of Intermediate Care Team, Urgent Community Response team, Referral Hub and the Virtual Ward.

Qualifications

  • Current NMC registration
  • Registered First Level Nurse
  • Educated to degree/diploma level Mentorship, ENB 998, practice assessor or equivalent
  • Demonstrates clear evidence of on-going professional development

Desirable
  • Independent Nurse prescriber
  • Leadership qualification

Other Requirements
Essential
  • Demonstrates the organisations values
  • Ability to work effectively as a team player under appropriate supervision, and as part of an MDT.
  • Insight into how to evaluate own strengths and development needs, seeking advice where appropriate.
