Job summaryCommunity Palliative Care Nurse Specialist, Band 6 (24 hours a week)
We are seeking a motivated and compassionate Community Specialist Palliative Care Nurse to join our dedicated specialist palliative care service in Gloucestershire. This is an exciting opportunity for an experienced nurse who is passionate about providing high-quality, person-centred care within a variety of community settings.
As a Community Specialist Palliative Care Nurse, you will play a key role in supporting patients with complex symptom management needs, enabling people to achieve their preferred place of care and supporting families through difficult and often life-changing circumstances.

Additional working pattern details: 9am - 5pm over 3 weekdays

Main duties of the jobYou will work collaboratively with GPs, district nurses, hospital teams, hospices, care homes and wider health and social care partners to provide specialist advice, assessment and support for patients, and those important to them, who have complex symptoms and needs.
Key responsibilities include:
- Assessing and managing patients with complex palliative and end of life care needs.
- Providing specialist symptom management advice and support.
- Developing personalised care plans in partnership with patients and families.
- Supporting advance care planning and decision-making conversations.
- Offering education and support to healthcare professionals across the system.
- Contributing to multidisciplinary team meetings and care coordination.
Supporting the delivery of high-quality, equitable specialist palliative care across Gloucestershire.

Job responsibilitiesWe are looking for someone who:
- Is a Registered Nurse with current NMC registration.
- Has significant post-registration experience, ideally within palliative care, oncology, community nursing or a related specialty.
- Demonstrates excellent communication and interpersonal skills.
- Is confident working autonomously whilst contributing effectively within a multidisciplinary team.
- Is someone who has critical thinking skills, uses initiative and is keen to stretch themselves each day.
- Has a compassionate, patient-centred approach to care.
- Holds a full UK driving licence and has access to a vehicle for community visits.

Person SpecificationQualificationsEssential- Registered Nurse with appropriate experience in a relevant area of practice such as palliative care, community nursing or oncology
- Holds a full UK driving licence and has access to a vehicle for community visits.
Desirable- Leadership experience at band 6 level
KnowledgeEssential- Experience and knowledge of symptom management and fundamental strategies to manage this
- Experience of communicating complex and sensitive issues with patients, carers and professionals
Desirable- Palliative Care Qualification
- Experience of managing a caseload
Personal AttributesEssential- Demonstrate clear and advanced communication skills, written, verbal and non verbal
- Able to establish and maintain credibility with a diverse range of professionals and lay people
- Excellent planning and organizational skills
- Ability to form professional relationships within a multidisciplinary team
