These posts are office based and available full or part time, covering shifts across a 24/7 rota, across days, evenings, nights, weekends and bank holidays.

We are looking for compassionate and experienced Band 6 Registered Nurses to join our new Worcestershire 24/7 Palliative and End of Life Care Advice Line service. This is an exciting opportunity to be part of a countywide service providing timely advice, telephone triage, symptom management guidance, reassurance and care coordination for patients deemed to be in their last year of life, their families, carers, care homes and professionals.

Working as part of an integrated model with our Single Point of Access (SPA), Evening and Night Nursing, Neighbourhood Team and Specialist Palliative Care services with strong collaborative working with Primary Care and other system partners, you will help people remain in their preferred place of care and reduce avoidable crisis escalation and avoidable Emergency Department attendances or hospital admissions.

The role is clinically focused and will be co-located with other clinicians with the ability to escalate to more senior clinicians where appropriate. There will be only one clinician covering this phoneline so the ability to deliver the service confidently is essential.

Main duties of the job
In this role, you will provide telephone triage, clinical assessment and advice to patients deemed to be in their last year of life, as well as their families, carers, care homes and healthcare professionals. You will offer symptom management guidance, reassurance and end of life care advice, helping to prevent crisis escalation and support people to remain in their preferred place of care and preferred place of death.

You will coordinate and navigate care across services, making appropriate referrals or escalations to Neighbourhood Teams, Urgent Community Response, GP out of hours, Community Specialist Palliative Care Teams or other services when needed. You will work as part of the SPA, Evening and Nights and Specialist Palliative Care model, with access to clinical supervision, specialist training and specialist advice.

A typical shift will involve responding to calls, assessing need, giving patients or their families time to talk and provide reassurance, providing compassionate advice, documenting contacts and supporting safe, timely care decisions.

Job responsibilities
The post holder will contribute to the delivery of a safe, responsive and clinically effective 24/7 Palliative and End of Life Care Advice Line for Worcestershire. They will work within agreed pathways, policies and escalation processes to support timely, person-centred care for patients, families, carers and professionals.

Key responsibilities include maintaining accurate clinical records, supporting care coordination, contributing to audit and service evaluation, participating in training, supervision and competency development, and working collaboratively with SPA, Neighbourhood Teams, Urgent Community Response, primary care, hospices and community specialist palliative care services.

The role requires compassionate communication, sound clinical judgement, effective risk assessment and the ability to work autonomously while seeking support appropriately.

Qualifications
Essential

• Registered Nurse (RN1)
• Graduate Nursing Qualification at Academic Level 6 - or working towards.
• Preceptorship training
• Evidence of on-going continuing professional development

• District Nursing Specialist Practitioner Qualification
• Practice educator

Experience
Essential

• Significant post registration experience in a clinical setting - able to demonstrate transferability of experience to community setting
• Experience of management of caseloads or wards and organisation of staff
