Bryngwyn Surgery is looking for an experienced and enthusiastic Practice Nurse to join our friendly and supportive team.

The role will involve providing a broad range of nursing services within general practice, including long-term condition management, health promotion, vaccinations and immunisations, cervical screening, wound care and other treatment room duties.

We are looking for someone who is confident working independently, but who also enjoys being part of a close-knit multidisciplinary team. Good communication, organisation and a patient-centred approach are essential.

This is a varied role with the opportunity to contribute to the continued development of nursing services within the practice. We offer a supportive working environment and are happy to discuss hours and working patterns with the right candidate.

Main duties of the job
The Practice Nurse will work as part of the wider practice team to provide safe, effective and patient-centred nursing care.

Main duties will include:

• Long-term condition reviews and management, including diabetes, asthma, COPD and cardiovascular disease.
• Cervical screening in accordance with current national guidance.
• Adult and childhood immunisations and vaccinations.
• Wound care, dressings and suture or clip removal.
• Health promotion, lifestyle advice and preventative care.
• NHS health checks and other routine clinical assessments.
• Supporting patients with smoking cessation and other health improvement initiatives.
• Carrying out appropriate clinical observations and tests.
• Maintaining accurate and contemporaneous clinical records.
• Working within agreed practice protocols, PGDs and relevant professional guidance.
• Identifying and escalating clinical concerns appropriately.
• Contributing to infection prevention and control, clinical governance, audits and quality improvement work.
• Supporting the practice in achieving relevant contractual and quality targets.
• Working closely with GPs, nurses, healthcare assistants, pharmacists and the wider practice team to provide coordinated care.
• Maintaining professional development and ensuring mandatory training and clinical competencies remain up to date.

Job Purpose

The Practice Nurse will work as part of the multidisciplinary practice team to provide safe, effective and patient-centred nursing care to the practice population.

The postholder will undertake a broad range of general practice nursing duties, working within their professional competence and in accordance with practice policies, current clinical guidance and NMC requirements.

The role will include long-term condition management, preventative care, immunisations, screening, treatment room duties and health promotion.

Main Responsibilities
Clinical Care
The Practice Nurse will:

• Undertake long-term condition reviews, including diabetes, asthma, COPD and cardiovascular disease, according to competence and practice protocols.
• Provide cervical screening in accordance with the current screening programme and relevant guidance.
• Administer adult and childhood vaccinations and immunisations under appropriate Patient Group Directions or prescriptions.
• Provide wound care, dressings, suture and clip removal.
• Undertake appropriate clinical observations and investigations, including blood pressure, pulse, oxygen saturation, urinalysis and other tests within competence.
• Provide health promotion and lifestyle advice, including smoking cessation, weight management, alcohol advice and cardiovascular risk reduction.
• Support disease prevention, screening and early identification of health problems.
• Provide travel health advice and vaccinations where this forms part of the practice service and the postholder is appropriately trained.
• Recognise changes in a patient's condition and escalate concerns appropriately.
• Provide appropriate advice and safety-netting to patients.
• Refer patients to other members of the clinical team or external services where appropriate.

Long-Term Condition Management
The postholder will support the effective management of patients with long-term conditions by:

• Carrying out structured reviews.
• Monitoring clinical indicators and treatment.
• Supporting patients to understand and manage their condition.
• Identifying patients who require further clinical review.
• Working collaboratively with GPs, pharmacists and other clinicians.
• Supporting the practice in achieving relevant contractual, clinical and quality improvement requirements.

Infection Prevention and Control
The Practice Nurse will:

• Follow infection prevention and control policies and procedures.
• Maintain high standards of cleanliness and aseptic technique.
• Support appropriate vaccine and cold-chain management.
• Ensure clinical equipment is used, stored and maintained appropriately.
• Report infection control concerns promptly.
• Contribute to infection prevention audits and improvement work where required.

Clinical Governance and Quality
The postholder will contribute to the practice's clinical governance arrangements, including:

• Maintaining accurate, clear and contemporaneous clinical records.
• Working in accordance with agreed clinical protocols and evidence-based guidance.
• Participating in clinical audit and quality improvement.
• Reporting incidents, near misses and clinical concerns appropriately.
• Contributing to learning from significant events, complaints and patient feedback.
• Participating in meetings and practice development activity where appropriate.
• Maintaining awareness of safeguarding responsibilities and acting on concerns in accordance with local procedures.

Medicines Management
The Practice Nurse will:

• Work within agreed Patient Group Directions, protocols and prescribing arrangements.
• Ensure medicines and vaccines are stored and handled safely.
• Maintain appropriate stock levels and monitor expiry dates.
• Report medication or vaccine incidents in accordance with practice procedures.

Communication and Team Working
The postholder will:

• Work collaboratively with GPs, nurses, healthcare assistants, pharmacists, administrative staff and other members of the wider primary care team.
• Communicate sensitively and effectively with patients, carers and colleagues.
• Support patients to make informed decisions about their care.
• Recognise communication needs and arrange appropriate support, including interpretation or accessible communication where required.
• Maintain professional boundaries and confidentiality at all times.

Professional Development
The Practice Nurse will:

• Maintain current NMC registration.
• Work within the NMC Code and their individual scope of professional competence.
• Maintain evidence of continuing professional development.
• Complete required statutory and mandatory training.
• Maintain clinical competencies relevant to the role.
• Participate in appraisal and development discussions.
• Identify areas where additional training or supervision is required.

Confidentiality and Information Governance
The postholder must maintain confidentiality in relation to patients, staff and practice information and comply with relevant information governance, data protection and confidentiality requirements.

Clinical information must only be accessed, recorded or shared where there is a legitimate reason to do so.

Qualifications
