Are you passionate about diabetes care and looking to develop your specialist skills?

We have an exciting opportunity for a Band 6 Community Diabetes Specialist Nurse to join our friendly, supportive nurse-led team.

Working across community and primary care settings, you will provide specialist assessment, education and support to adults living with diabetes, helping them to improve their health and reduce the risk of diabetes-related complications.

The role includes providing specialist advice and guidance to primary care, supporting the initiation and optimisation of glucose-lowering therapies including insulin, delivering patient and healthcare professional education, and working collaboratively with colleagues across primary, community and secondary care.

You'll also have the opportunity to contribute to service improvement and transformation, helping us to improve outcomes and reduce variation in diabetes care across the locality.

We are looking for an enthusiastic and motivated nurse with an interest in diabetes who enjoys autonomous working, collaborative care and supporting both patients and healthcare professionals.

Main duties of the job

• To provide a specialist nursing service to adults living with diabetes as part of a small nurse-led team.
• Deliver care, education and support to people living with diabetes in primary care.
• Help those patients living with diabetes to reduce risks of acute and long-term complications associated with diabetes.
• Deliver structured education to those newly diagnosed with type 2 diabetes.
• Deliver education to healthcare professionals.
• To actively participate in the planning and delivery of diabetes care, as part of the wider multi-professional team.
• To participate in service improvement and transformation initiatives to improve diabetes outcomes and reduce variability of care delivered across the locality.

Qualifications

• Registered Nurse at level 1
• Current NMC registration
• Post registration course in Diabetes
