Job summaryThis is an exciting opportunity for an Extended Clinical Practitioner (ECP) to join our Hospital at Home service, providing responsive, hospital-level care to people in their own homes.
Working as part of a supportive multidisciplinary team, you will use your advanced assessment and clinical decision-making skills to assess and manage people with acute illness, frailty and complex needs. The role offers real variety, with opportunities to undertake comprehensive assessment, diagnostics and treatment in the home, working closely with senior clinicians and wider health and care services to help people receive the right care in the right place.
Hospital at Home is continuing to develop across Hampshire and the Isle of Wight, and we are looking for clinicians who want to be part of that journey. You will have the opportunity not only to deliver high-quality clinical care, but also to help shape pathways, develop new ways of working and contribute to the ongoing development of the service.
If you enjoy autonomous practice, clinical challenge, working across traditional service boundaries and making a tangible difference to people's experience of urgent and acute care, we would love you to join us.
