Job summaryWe are looking for a Diabetes Nurse to join our Long Term Conditions team.
In this role you will provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team. The duties will include all tasks normally undertaken by an experienced RGN and in addition any roles agreed between the nurse and the doctors as appropriate, having regard to current training.
The focus of the role is both the delivery of evidence-based practice for patients presenting with long term conditions and the provision of preventative health care to the practice population. As an autonomous practitioner you will be responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. You will work collaboratively within the general practice team and the wider community teams to meet the needs of patients, support the delivery of policy and procedures and provide leadership where required.

Main duties of the job- Provide a holistic approach to individuals that includes analytical and judgement skills in triaging telephone calls to assess and evaluate the information given, use appropriate further questioning, diagnose a problem/illness and reach a clinical conclusion in accordance with agreed PGDs, guidelines and protocols.
- Assist in the diagnosis and management of acute presentations and attend to the patient's response to this experience of illness.
- Takes a lead in Diabetes management, undertaking the establishment and running of disease management clinics and provides specialist nursing care for the patients or communities that the practice serves.
- Manages programmes of care for patients with chronic diseases by planning, providing and evaluating care under agreed local/national guidelines, protocols and patient group directions.
- Provide the full range of nursing services, including venepuncture, ECG measurement, blood pressure measurement, urinalysis, vaccinations and immunisations (children and adults including travel), baby clinic, injections, cervical smears, swabs, travel services, health checks, assessment of risk factors, lifestyle advice, weight management, smoking cessation, ear care, suture removal, wound care etc

Person SpecificationQualificationsEssential- Current registration with NMC
- Educated to diploma level (Practice Nurse Qualifications or equivalent)
- Qualified practitioner / completed a minimum of one recognised level 6 Autonomous Practice Module relating to the diagnosis and management of either injuries or illness
- Up-to-date skills and experience in the full range of practice nursing services, including the nurse-led management of patients with chronic diseases.
Desirable- Non-Medical prescribing Course
ExperienceEssential- Post Registration experience
- Good IT skills -- ability to utilise appropriate electronic systems to accurately record data.
- Ability to work autonomously without supervision.
- Ability to meet deadlines and to work under pressure.
- Ability to adapt to the demands of a constantly changing environment.
- Aware of accountability of own role and other roles in a nurse led service.
