Patients are often seen within two hours of referral, enabling timely interventions that improve outcomes and keep people safely at home. You will work autonomously while being supported by a skilled multidisciplinary team including Consultant Practitioners, Advanced Clinical Practitioners, Therapists, GPs and experienced nursing colleagues. Main duties of the jobAs a Registered Nurse or Allied Health Professional within the Frailty Hospital at Home team, you will provide rapid assessment, treatment and ongoing case management for patients with frailty, complex health needs and long term conditions. Working as part of a multidisciplinary team, you will deliver high quality, person centred care that helps prevent unnecessary hospital admissions and supports patients to remain safely at home. Key responsibilities include undertaking comprehensive clinical assessments, developing and reviewing care plans, coordinating patient care, managing complex caseloads and delivering evidence based interventions. You will work closely with patients, carers, GPs, community services and wider healthcare partners to ensure seamless care and positive patient outcomes. The role also involves supporting clinical triage, responding to urgent referrals, providing clinical advice, maintaining accurate patient records and contributing to service improvement initiatives. As a senior clinician, you will support and mentor junior staff, promote best practice and help ensure the delivery of safe, effective and responsive care across the service.
