Job summary
We are looking for a compassionate and skilled Senior Mental Health Primary Care Practitioner to join our dynamic multidisciplinary team. This role offers the opportunity to make a meaningful difference by supporting adults with mental health needs directly within primary care settings, helping people access timely, person centred support closer to home.

Working as part of our Primary Care Network, you will use your clinical expertise to triage referrals, complete comprehensive assessments, and deliver brief therapeutic interventions. You'll work collaboratively with GPs, practice teams, and secondary care services to ensure safe, seamless and recovery focused care. Your ability to use clinical judgement, assess and manage risk, and communicate effectively will be essential in this fast paced and rewarding role.

You'll be supported through regular supervision, MDT discussions and opportunities for professional development. Main duties of the job
You will triage referrals using clinical judgement, completing timely assessments, formulations and brief interventions for adults presenting with mental health needs in primary care. You will assess and manage risk, develop risk management plans and adjust these in line with service user needs. [B6 JD SMHP | Word]

The role involves working closely with GPs, practice teams and secondary mental health services, including attending MDT meetings and liaising with partner agencies. You will coordinate care, support signposting to appropriate services and contribute to safe, effective planning for ongoing support. Accurate and timely record keeping in primary care systems is essential. [B6 JD SMHP | Word]

You will act autonomously while accessing regular supervision, reflective practice and clear escalation pathways. You may provide advice and supervision to junior staff such as social prescribers or peer support workers, and contribute to service improvement, policy implementation and maintaining service standards. We are looking for someone confident, compassionate and skilled in communication, with strong risk assessment abilities, effective teamwork and the capacity to work independently in a busy primary care environment. [B6 JD SMHP | Word]

Across Hampshire, our community hospitals provide inpatient rehabilitation as a step down from acute care, and our dedicated teams also staff Treetops Sexual Assault Referral Centre in Portsmouth, offering expert, compassionate support.

Person Specification
Qualifications
Essential
  • Registered professional qualification as a Mental Health Nurse, Social Worker or Occupational Therapist
  • Evidence of CPD (continued professional development)
  • Ability to fulfil travel requirements
