Job SummaryWe are looking for a dedicated Mental Health Nurse to join us on our Primrose Inpatient Ward, based at the Western Community Hospital in Southampton. Primrose Ward is a 14 bed OPMH functional ward. The ward provides intensive care, acute assessment and treatment for older people experiencing functional mental health needs, such as depression, bipolar disorder or schizophrenia.
The primary purpose of a Band 5 Nurse on an Older Persons Ward is to provide direct, compassionate, and effective nursing care to elderly patients, many of whom may have multiple medical conditions, dementia, mobility issues. The nurse is integral in supporting the patient's dignity, safety, and comfort while ensuring optimal care in line with evidence- based practices.
Main Duties of the Job- Assessment and care planning - Conduct comprehensive assessments, including physical, mental and emotional needs. Develop and implement individualised care plans
- Safely administer medications, monitor for side effects & document treatments accurately
- Provide basic nursing, such as wound management, catheter care, feeding support & personal hygiene assistance
- Observe patient's physical and mental health conditions, including signs of deterioration and promptly report any concerns to senior nursing staff
- Apply person centred care, helping to maintain the dignity of patients with memory loss or confusion
- Work closely with families and carers to ensure a holistic approach to care
- Create a safe and calming environment to reduce the risk of falls, confusion and distress
- Being responsible for ensuring that patients are safeguarded; includes identifying signs of elder abuse or neglect and following the correct protocols to protect the patients

QualificationsEssential- Registered Mental Health Nurse with NMC registration.
- Computer skills, knowledge of RiO systems.
