Job summary
An exciting opportunity has arisen to be part of the Petersfield and Bordon Community Occupational Therapy service, which works within the larger integrated care model.

You will be joining a leading organization that is both forward thinking and innovative. The applicant is afforded structured professional supervision and an annual appraisal.

This Occupational Therapist position is designed to contribute to avoiding hospital admission, rehabilitation/re-ablement, facilitating and maintaining patients in their own homes. Clients accessing this service will be predominantly (but not exclusively) in the elderly care group (over 65). This post offers the opportunity to work closely with all members of the MDT both in primary, secondary care, Adult Services as well as working closely with care/nursing home staff.

At present, the Trust is unable to offer Skilled Worker visa sponsorship for this post. Applicants must have the right to work in the UK at the time of application.

Main duties of the job
  • To provide advanced Occupational Therapy to the Community Therapy Team/Community Hospital Team, and to provide support to the Integrated Community Care team leaders in ensuring that the team contributes to the implementation of Hampshire & Isle of Wight Healthcare Clinical Governance Strategies.
  • Undertake Occupational Therapy assessments of patients, providing a clinical diagnosis, development and delivery of individualised treatment programmes.
  • To support the case management process by providing expert clinical advice and therapy to patients with a variety of complex conditions living in the community who are at risk of deteriorating health that may result in declining quality of life or avoidable hospital admission to secondary care
  • To contribute to partnership working with partners in Primary and Social care to support Integrated Care team model which identifies, and case manages those patients needing complex chronic disease management or palliative care supporting the needs of the local community.
  • Negotiate and agree with the patient, carers and other care professionals, individual roles and responsibilities with actions to be taken and outcomes to be achieved, referring on to other services or professionals as appropriate.
  • Use assessment tools/skills that will ensure an appropriate level of therapy intervention so that patients who present with highly complex needs are timely referred to the appropriate clinician.
  • To work at and travel between Petersfield and Bordon.

Job responsibilities
  • To contribute to partnership working with partners in Primary and Social care to support the Integrated Care Team (ICT)model which identifies, and case manages those patients needing complex chronic disease management or palliative care supporting the needs of the local community.
  • To participate in professional leadership roles for AHPs in the Community Therapy Team/ Community Hospital Team(s). Act as a clinical exemplar within the team and locality/business unit.
  • Provide effective leadership and management to staff which promotes high performance standards both individually and as a team, in the achievement of the Trust's objectives and priorities. The Trust's success will be dependent on all managers playing an active role to make sure the existing areas of good employment practice are universally embedded within the organisation.
  • As a supervisor, delegate tasks to assistant practitioners and junior therapists relevant to their skill level and competence
  • Act as a Professional Leader and Role Model for AHPs and other professionals within the Community Therapy Team(s) and Integrated Community Teams (ICTs), developing knowledge of ideas and work practice, promoting this through appropriate attitudes and behaviour.
  • Assist in operational management for the Community Therapy Team undertaking; recruitment, induction, regular supervision, appraisals, performance monitoring and coordination of the team, their clinical practice and record keeping.
  • Undertake prioritisation of referrals and management of any waiting list.
  • Be able to balance team, patient related and professional activities effectively and efficiently in accordance with the Community Therapy Team and Integrated Care teams ICT requirements.
  • Deputise for team leader(s) as required.
  • As a lone worker be fully aware of and compliant with both the Trust and Departmental Lone Working Policies.
  • To be professionally responsible and accountable for all aspects of your own and your team's work including the management of patients care.

Person Specification
Qualifications
  • Essential: Registered Allied Health Professional, educated to Degree level or Diploma equivalent.
  • Desirable: Practice Educator.

Experience
  • Essential: Relevant specialist/clinical experience and competence including: Complex therapy/care co-ordination/manual handling and posture management; Proactively manage complex conditions; Knowledge of managing cognitive impairment and mental well-being; Supporting self-care, self-management and enabling independence; Developing programmes of therapy, providing advice concerning treatment of patients; Professional practice and leadership; Experience of inter-agency and partnership working; Experience in carrying out clinical audit and risk assessment, outcome measurement; Proven skills in supervision, and contribution to service development; Strong IT Skills -- including excel and power-point; Good Presentation skills / public speaking; Ability to work autonomously without supervision; Ability to meet deadlines and work under pressure; Ability to adapt to the demands of a constantly changing environment; Responsive and flexible; Innovative approach to improving patient experience; Promotes service improvement; Seeks opportunity to provide medium -- long term value addresses inefficiencies quickly; Evaluates and streamlines ways of working; Proactive, solutions focused attitude; Focuses self and team to deliver results; Provides clear goals and objectives; Promotes cohesion and team working; Works collaboratively with teams within the Trust and across the Local Health Economy; Resolves conflict effectively; Ability to travel across community & PSEH Geography to deliver services affectively; Assertive, honest and open resolving problems.
  • Desirable: Experience of practice within community settings; Multi-disciplinary working; Specialised treatment programmes; Adult services and/or voluntary sector.
