Job IntroductionAs a Band 6 Physiotherapist, you will join our Early Supported Discharge (ESD) team based at Gravesham Community Hospital in Gravesend. This is a rewarding community role, supporting patients with neurological conditions, including stroke, to transition safely from hospital to home. You will play a key role in delivering high quality rehabilitation within patients own environments, helping them regain independence and confidence. This is a varied role where you can follow the patient journey, making a real and measurable impact on recovery outcomes. You will be part of a friendly and experienced multidisciplinary team, working closely with Occupational Therapists, Speech and Language Therapists, Nurses and support staff to deliver coordinated, patient centred care.
