Are you passionate about making a meaningful difference to the lives of children, young people, and their families?

We're looking for a caring and proactive Care Navigator to join our Neurodevelopmental Pathway team, providing needs-led support and tailored interventions for families navigating autism and ADHD with a particular focus on those with an ADHD diagnosis.

Based at one of our hub locations across B&NES or Wiltshire (depending on your preferred base), this role offers a mix of hub-based work, visits across the region, and the flexibility for some remote working. You'll play a key part in helping families access the right support at the right time, guiding them through their journey with compassion, clarity, and confidence.

This is a full-time role working 37.5 hours per week.

Base: Derby Court, White Horse Business Park, Trowbridge, BA14 0XQ

Main Responsibilities:

• Supporting families and other professionals to understand how and when to refer into the service for medication to support ADHD/ sleep.
• Gathering information from families, children and young people to support the assessment process.
