Job summaryAre you an experienced Registered Nurse with a passion for leadership, rehabilitation and delivering outstanding patient care? HCRG Care Group is proud to have been chosen by Medway Council to deliver the Intermediate Care and Reablement Service across the Medway community from 1 October. As we launch this exciting new service, we are looking for a Clinical Lead Nurse to play a pivotal role in supporting both our Home First and Bedded Intermediate Care pathways. This is a unique opportunity to join a service at the very start of its journey and help shape how care is delivered across Medway. Working alongside senior therapy colleagues and multidisciplinary partners, you will provide clinical leadership, support service development and help ensure local people receive high-quality, person-centred care that promotes independence and positive outcomes.
