Job summary
Make a Difference at the Front Door of Community Healthcare

Every referral is an opportunity to improve someone's journey through care. As a Clinical Triage Practitioner within our Community Health Single Point of Access (SPA), you will play a pivotal role in ensuring patients receive the right care, in the right place, at the right time. Using your advanced clinical judgement and knowledge of community and urgent care services, you will help prevent unnecessary hospital admissions, support safe and timely discharges, and improve outcomes for patients across our communities.

This is an exciting opportunity for an experienced clinician who thrives in a fast-paced environment and is passionate about delivering high-quality, patient-centred care through effective assessment, coordination and system navigation.

About the Role
As a Clinical Triage Practitioner, you will provide expert telephone-based clinical triage and assessment within the Community Health Single Point of Access. Working autonomously as part of a multidisciplinary team, you will assess referrals from health and social care professionals, determine clinical priority, and coordinate access to appropriate community services.

You will act as a key link across health, social care and voluntary sector services.
