Job summaryAs a Band 5 Community Nurse, you will join our valued Community Nursing Team based at Sheppey Community Hospital, providing care across Sheppey, Sittingbourne and the surrounding villages. This is a rewarding opportunity to deliver high quality nursing care where it matters most, within patients' homes and local communities. You will support patients with a wide range of healthcare needs, helping them maintain independence, manage long-term conditions and avoid unnecessary hospital admissions.
