Job summary
Package Description: Are you passionate about providing compassionate care and supporting people to live as independently as possible? Do you want a rewarding role where you can make a real difference to patients in your local community every day?

We are looking for a caring, motivated, and enthusiastic Band 3 Community Rehabilitation Assistant to join our Intermediate Care Team.

About the Role
Working across Surrey Heath and Farnham, you will support people in their own homes following illness, injury, hospital admission, or a decline in function. As part of a multidisciplinary team, you will help patients regain confidence, improve their independence, and achieve their rehabilitation goals, enabling them to remain safely in the place they call home.

This is a rewarding role that combines care and rehabilitation, placing the patient at the centre of everything we do. You will work alongside Physiotherapists, Occupational Therapists, Nurses, and other healthcare professionals to deliver high-quality, person-centred care.

What You'll Be Doing
As a Community Rehabilitation Assistant, you will:
  • Visit patients in their own homes across Surrey Heath and Farnham, supporting them to maximise their independence and achieve their rehabilitation goals following illness, injury, hospital admission, or a decline in functional ability.
  • Deliver person-centred care and rehabilitation in accordance with individualised rehabilitation plans developed by the therapy team in partnership with patients and their families.
  • Support patients with activities of daily living, including washing, dressing, personal care, simple meal preparation, and medication support where appropriate and within agreed competencies.
  • Assist patients with exercise programmes and rehabilitation activities prescribed by Therapists, encouraging participation, confidence, and progression towards agreed goals.
  • Empower individuals to do as much as possible for themselves, promoting independence, dignity, and wellbeing at every opportunity.
  • Monitor and document patient progress using a tablet, ensuring records are maintained accurately and contemporaneously while working in the community.
  • Recognise and report any changes in a patient's physical, emotional, cognitive, or functional wellbeing.
  • Work collaboratively with Nurses, Physiotherapists, Occupational Therapists, and other healthcare professionals to deliver safe, effective, and coordinated care.
  • Contribute to the delivery of safe, effective, compassionate care in line with organisational policies, procedures, delegated responsibilities, and competency frameworks.
  • Undertake administrative duties required to support patient care and the smooth running of the service.
  • Maintain and develop the knowledge, skills, and behaviours required to provide high-quality care that responds to the changing needs of patients and the wider healthcare system.

What We Are Looking For
We are seeking individuals who:
  • Are passionate about supporting people to regain independence.
  • Have a caring, compassionate, and respectful approach.
  • Enjoy building positive relationships with patients and colleagues.
  • Have excellent communication and interpersonal skills.
  • Can work independently while also being an effective team member.
  • Are adaptable, reliable, and motivated to deliver high-quality care.
  • Have previous experience in a health or social care setting (desirable but not essential).

Learning and Development
As part of our commitment to lifelong learning, we offer the Care Certificate to all eligible unregistered healthcare professionals. This forms the foundation of your education and training with us and is an integral part of our comprehensive induction programme.

The Care Certificate is a nationally recognised set of standards that defines the knowledge, skills, and behaviours expected of staff working within health and social care. We are committed to supporting ongoing professional development and career progression throughout your journey with us.

Why Join Us?
  • Make a genuine difference to people's lives every day.
  • Help patients remain independent and live well at home.
  • Be part of a supportive and experienced multidisciplinary team.
  • Access excellent training and development opportunities.
  • Work autonomously within the community.
  • Use modern digital technology to support patient care and reduce time spent in the office.
  • Contribute to preventing unnecessary hospital admissions and supporting timely discharge from hospital.

Working Pattern
We have a full-time (37.5 hours) position available.

The role includes working alternate weekends as part of the rota. Our service operates 7 days a week, with shifts shared fairly across the team.
  • Early Shift: 7:30am to 3:30pm
  • Late Shift: 1:00pm to 9:00pm

Person Specification
Qualifications
Essential
  • A caring and compassionate approach with excellent communication and interpersonal skills is essential. Previous experience in a health or social care setting is desirable but not essential.
