Description

Package Description:
Are you passionate about helping people stay active, independent and safe in their own homes?

We are looking for an enthusiastic Healthcare Assistant to join our Falls Prevention Team, working alongside Physiotherapists, Rehabilitation Assistants and other members of the wider multidisciplinary team.

Our Falls Prevention Service supports people who have experienced falls or are at risk of falling, helping them improve their strength, balance, confidence and independence. Through practical support, wellbeing interventions and rehabilitation programmes, we help people remain safely at home and reduce the need for unnecessary hospital attendance.

This is a varied and rewarding role where you'll spend time supporting patients in their own homes, assisting with rehabilitation programmes, carrying out wellbeing visits and helping to deliver group-based strength and balance activities.

The position is for 22.5 hours per week and offers the opportunity to make a genuine difference within your local community.

Please note: a full UK driving licence and access to a car are essential as travelling within the community forms a key part of this role.

Package Description
As a Healthcare Assistant - Falls Prevention Team, you'll be part of our valued community team based at Livingstone Community Hospital in Dartford, North Kent.

You'll play an important role in helping people remain safe, independent and confident within their own homes, supporting a service that focuses on preventing falls and reducing unnecessary hospital admissions.

You will feel valued within HCRG Care Group, receiving access to exclusive rewards and benefits including:
  • A salary of £24,875 with access to our Group Pension
  • Free tea, coffee and milk at your base location
  • Membership of My Reward Hub, giving you access to discounts on everyday purchases such as grocery shopping, as well as cashback and voucher offers
  • Access to your wages as you earn them to help cover life's emergencies and avoid overdraft fees or high interest rates
  • Online and face-to-face help with your mental and physical wellbeing, from healthy recipes and activity challenges through to post-trauma support, legal, debt and life management help, career coaching and counselling
  • Access to eLearning, bespoke career pathways and opportunities for continuing professional development through our 'Outstanding' learning and development team, The Learning Enterprise
  • An open, just culture where you're encouraged to share ideas and help improve the way we deliver care

Main Responsibilities
As a Healthcare Assistant within the Falls Prevention Team, you will:
  • Support the delivery of strength and balance classes
  • Visit patients within their own homes to support exercise programmes and wellbeing interventions
  • Assist with the review of mobility and daily living equipment
  • Support rehabilitation programmes developed by Physiotherapists and Rehabilitation Assistants
  • Encourage and motivate patients to achieve their rehabilitation goals and maintain independence
  • Provide information and practical guidance to patients, relatives and carers
  • Participate in health promotion and wellbeing activities
  • Monitor and report changes in a patient's condition or circumstances to the appropriate clinician
  • Maintain accurate and timely patient records
  • Provide administrative and clerical support to the team where required
  • Attend team meetings, training and development activities
  • Work independently when undertaking delegated visits while remaining part of a supportive multidisciplinary team
  • Promote patient safety, dignity and wellbeing at all times

The Ideal Candidate
You'll be a caring and motivated individual who enjoys working directly with people and helping them achieve greater independence.

We're looking for someone who can build positive relationships with patients, carers and colleagues, while remaining organised and adaptable within a busy community service.

You will have:
  • NVQ Level 2 in Care or an equivalent qualification
  • Previous experience within a health, social care or rehabilitation environment
  • Strong communication and interpersonal skills
  • The ability to work effectively as part of a team and independently when required
  • Good organisational and time-management skills
  • A compassionate and patient-centred approach
  • An understanding of the importance of dignity, privacy and confidentiality
  • Good IT skills and confidence using electronic systems
  • The ability to recognise concerns and escalate information appropriately
  • A willingness to learn and develop new skills

Experience within rehabilitation, community healthcare, therapy services or group activity delivery would be advantageous but is not essential, as training and support will be provided.

A full UK driving licence and access to a vehicle for work purposes are essential for this role.
