Description

Job summary
Package Description: As a Healthcare Assistant, you will join our valued Gravesham Central Community Team based at Gravesham Community Hospital. This is a rewarding opportunity to deliver care where it matters most, in peoples homes and local clinics, helping individuals stay independent and supported within their own communities. This role is ideal for someone who enjoys working out in the community rather than being based in one location all day. You will build meaningful relationships with patients and play a key role in supporting their health and wellbeing in a more personal, one to one setting. You will be part of a supportive, close knit community nursing team that works together to deliver high quality, compassionate care across the Gravesend area.

You will be joining a welcoming and experienced community nursing team who are passionate about delivering personalized care closer to home. The team supports each other, shares knowledge and encourages autonomy, giving you the confidence to manage your own visits while knowing support is always available. Alongside this, you will receive:
  • A salary of £24,785 with access to our group pension
  • Free tea, coffee and milk at your base location
  • Membership of My Reward Hub, giving you access to discounts on everyday purchases, cashback and voucher offers
  • Access to your wages as you earn them to help manage unexpected costs
  • Online and face to face wellbeing support including mental health, financial advice, counselling and coaching
  • Access to eLearning, career pathways and ongoing development through our Outstanding Learning Enterprise team
  • An open and inclusive culture where your ideas are welcomed and supported through innovation funding

Main Responsibilities
In this community based role, you will support the delivery of nursing care to patients in their own homes and local clinics. This is a varied and active position where no two days are the same. You will play a vital role in helping patients manage their health, recover from illness and maintain independence. You will work closely with registered nurses and the wider multidisciplinary team, carrying out delegated care and developing your clinical skills. You will also have the opportunity to work independently, planning your visits and managing your time effectively across the local area. Key responsibilities include:
  • Supporting patients with delegated nursing care in their own homes
  • Carrying out clinical tasks such as dressings and glucose monitoring following training and competency sign off
  • Supporting holistic, evidence based care that meets each patients individual needs
  • Maintaining accurate patient records using electronic systems
  • Assisting with administrative tasks to support the team
  • Planning and organizing your own daily visits across the community

This role requires travel across the local area, so a UK driving license and access to a car is essential.

Person Specification
Essential
  • NVQ Level 2 or 3 in Health and Social Care, or equivalent experience
  • A genuine interest in community based care and supporting patients in their own homes
  • Full UK driving license and access to a car
  • Good communication and interpersonal skills
  • Ability to work independently and manage your own workload
  • Basic IT skills including Microsoft Office

Desirable
  • Previous experience working in a community or domiciliary care setting
  • Experience supporting a wide range of patient needs in a non hospital environment
  • Confidence working autonomously in the community

About the Company
We change lives by transforming health and care. Established in 2006, we are one of the UK's leading independent providers of community health and care services, working with health and care commissioners and communities to transform services with a focus on experience, efficiency, and improved outcomes. We deliver and transform adult and children community health services, primary care services including urgent care, sexual health, dermatology and MSK services as well as adult social care and wellbeing services. Across England, we support communities of many millions and directly help more than half a million people each year, guided by our simple values: we care, we think, we do. We're committed to equal opportunities and welcome applications from a broad, diverse range of people who want to join our team. We're a Disability Confident Committed company, so we work to provide facilities, work environment adjustments and technical solutions to be as inclusive of everyone. While it doesn't happen often, sometimes a role is very popular and we'll need to close it earlier than the date we've shown here. If you're keen to join our team, we'd love to hear from you so please apply as soon as you can. As you'd expect, safeguarding and protecting the children, young people and vulnerable adults that we work with is of the utmost importance so we have policies and procedures in place to promote safeguarding and safer working practices and everyone who joins the team is subject to a safer recruitment process, including the disclosure of criminal records and vetting checks
