Main duties of the jobThe Practice Nurse is essential in providing comprehensive nursing care, focusing on patient assessment, treatment, and management within a primary care setting. This role involves a variety of clinical tasks including chronic disease management, dressings, immunisations, and health education. The Practice Nurse will work under the guidance and supervision of our Lead Nurse, contributing to the practice's goals by delivering high-quality patient care and participating in health promotion initiatives.
The Practice Nurse is expected to perform clinical procedures and manage patient follow-ups, ensuring the delivery of care is in accordance with the latest clinical guidelines and within the scope of practice. This position requires excellent nursing skills, a patient-centred approach to care, and the ability to work effectively within a multi-disciplinary team to improve health outcomes.
About usOur practice is located in Oldbury and we have over 9000 patients.
The surgery is made up of a diverse clinical team consisting of GP's Nurses and a HCA. We are also a training practice so have trainee GP's. Our Clinical Teams, alongside our Admin, Reception and Pharmacy Teams all working in unison for the benefit of our patients.
We have 2 buildings in the same location, and ample car parking. There is an independent pharmacy located in part of our main clinical building.
Job responsibilities- Treatment room: Provide general treatment room nursing care such as wound management, vaccinations, phlebotomy, and chronic disease management.
- Treatment Planning: Implement treatment plans for a wide range of acute and chronic conditions in collaboration with the wider healthcare professionals.
- Home visits are an essential part of the practice nurse role.
- Health promotion and prevention: delivery a range of health care services that support health promotion and prevention including sexual health, and participate in public health programs and initiatives to promote wellness and disease prevention within the community.
- Patient Education: Deliver educational support to patients on health management, disease prevention, and lifestyle modifications.
- Specialised care: Organise and run specialised clinics relating to long term conditions management such as: asthma, COPD, Diabetes and other relevant nurse-led clinical areas.
- Care Protocols: Follow protocols for the management of chronic diseases and chronic disease registers such as diabetes, asthma, COPD, and cardiovascular disease.
- Patient Monitoring: Regularly review and monitor patient health status and adjust care plans as necessary.
- Vaccination Schedules: Administer vaccines according to national immunisation schedules.
- Patient Follow-Up: Ensure proper follow-up and record management for vaccinations and immunisations.
- Documentation: Maintain accurate and detailed patient records, ensuring confidentiality and compliance with GDPR.
- Data Management: Utilise Emis to enter patient data and retrieve patient information efficiently.
- Multi-disciplinary Teamwork: Work closely with GPs, Lead Nurse, and other healthcare professionals to ensure cohesive patient care and service delivery.
- Professional Development: Engage in continuous professional development to keep up-to-date with the latest healthcare practices and innovations.
- Students and supervision: Facilitate learning experiences of other clinicians, including trainees, provide supervision, and promote shared learning.
- Clinical Audits: Participate in clinical audits and contribute to quality improvement meetings to enhance care standards.
- Feedback Implementation: Act on patient and staff feedback to improve clinical practices and healthcare outcomes.
- Infection Control: Adhere to strict infection control protocols, including the use of Personal Protective Equipment (PPE) and hand hygiene practices.
- Cold chain and drug stock: Safeguard and contribute to effective cold chain compliancy, ensure appropriate systems are in place and adhered to in regard to drug stock safety and rotation.
Person SpecificationExperienceEssential- Previous experience as a Practice Nurse in a primary care setting, with a strong understanding of public health and preventative care measures.
- Competent in using Emis & Docman Systems.
- Strong organisational skills, with the ability to manage multiple tasks efficiently and prioritize workload effectively.
- Proficient in a range of nursing procedures, including immunisations, chronic disease management, wound care, and health screenings.
- Proven ability to work effectively both independently and as part of a team.
- Excellent communication and interpersonal skills, essential for educating patients and interacting with other healthcare professionals.
Desirable- Commitment to collaborative practice and continuous professional development.
- Ability to convey complex information clearly and sensitively, adapting the message for diverse audiences.
QualificationsEssential- Registered General Nurse
- Full NMC registration
- Qualifications/Certificates in cervical screening, immunisation administration, relevant to chronic disease management, wound care, and other areas relevant to post
- Recognised qualification to diploma level or above
- A demonstrable commitment to professional development
- Experience of working in a GP Practice / Primary Care
