Job summary
We are currently seeking a P/T GP 2 days per week to join Horsman's Place Surgery, comes with a range of benefits, including professional development opportunities, pension scheme, and generous holiday entitlement. You will also have the chance to work alongside a diverse and experienced team of healthcare professionals, including nurses, pharmacists, and other specialists, to provide a truly integrated approach to patient care.

Main duties of the job
As a GP in our practice, you will be responsible for providing primary medical services to the registered patients of the practice, both telephone & F2F ensuring the highest standards of care for all, undertaken in line with the relevant national primary care contractual regulations and guidelines. Provide care for both on-the day care for patients and pre-booked patients with long term conditions along with some admin tasks.

About us
We are friendly supportive training Practice in Dartford, Kent - We have approximately 9,600 patients. We provide a wide range of services, including asthma / COPD and diabetes clinics, childhood immunisations, Travel advice and minor surgery. We have a multidisciplinary team that includes GPs, Nurses, Physiotherapists, a Pharmacy Team. We also offer Phlebotomy.

Job responsibilities
  • Surgery consultations telephone consultations and E Consults, triaging, checking and authorising repeat prescriptions and dealing with queries, patient test results, patient medical reports and examinations on behalf of the
  • Provide health promotion and advice, signposting patients to appropriate services to maximise their health and social functioning and their ability to self-manage their care.
  • Be constantly mindful of the need for the provision of personalised care and population health, including reducing health inequalities and ensure that these are reflected in daily practice.
  • Make professional, autonomous decisions in relation to presenting patients, whether self-referred or referred from other health care workers within the organisation or external organisations.
  • Assess the health needs of patients with undifferentiated and undiagnosed problems taking appropriate action as necessary.
  • Screen patients for disease risk factors and early signs of illness taking appropriate action as necessary.
  • Record clear and contemporaneous consultation notes to agreed standards using IT systems.
  • Prescribe in accordance with locally agreed or nationally agreed prescribing formulary.
  • Work in accordance with all relevant clinical guidelines, pathways and protocols.
  • Responsible for tasks delegated to them, appropriate to their level of In-hours duty doctor sessions; to be agreed.
  • All General Practitioners are expected to participate in continuing professional development, annual GP appraisal, Revalidation and appropriate peer supervision and to remain up to date on professional developments as required by their professional body. GPs are actively encouraged to take their study leave entitlement in order to support the development needs identified in their PDP and appraisal. Undertake all requirements to remain on the Medical Performers List.
  • Ensure that they remain up to date with and implements all relevant good clinical practice guidance (e.g. NICE guidance).
  • Work in accordance with all relevant clinical pathways and protocols.
  • Active engagement with professional development lead, to promote patient safety and quality of service.
  • Participation in a range of clinical governance activities, including clinical audit and such quality improvement activities deemed necessary by external accrediting bodies.
  • Ensure that the requirements of the Care Quality Commission are reflected in his/her day to day practice and will maintain and develop the practices active management of CQC requirements.

Qualifications
Essential
  • Currently on a performers list and not suspended from that list or from the medical register
  • Enhanced Disclosure & Barring Service check
Desirable
  • Commitment to and experience of working as part of a multi-disciplinary and skill mixed team environment
  • Self-audit and reflection
  • Organised and efficient in record keeping and completion of paperwork
  • Time management being able to prioritise work and work under pressure
  • Computer literacy
  • Adaptability to change.
