Job summary
We are looking for an enthusiastic, compassionate and experienced Band 6 Cardiology Clinic Nurse to join our dynamic Community Cardiology Service. This is an exciting opportunity to become part of a supportive multidisciplinary team delivering high-quality outpatient and diagnostic cardiology services across a range of community settings.

The successful candidate will have a designated home-base clinic, providing continuity of care within their primary site, whilst supporting cross-site service delivery during periods of annual leave, sickness and operational demand. Working across our community clinics, you will undertake a varied role including cardiac diagnostics, nurse-led and consultant-led clinics, patient assessment, care planning and coordination of patient pathways, ensuring safe, effective and person-centred care.

This role requires excellent clinical knowledge, sound decision-making, strong organisational skills and the ability to work both independently and collaboratively within a multidisciplinary team. You will also support and develop junior colleagues, contribute to service improvement and uphold the Trust's values of being Kind, Expert, Collaborative and Aspirational.

Main duties of the job
  • As a Band 6 Cardiology Clinic Nurse, you will provide high-quality, patient-centred care across our Community Cardiology Service, delivering safe and effective outpatient and diagnostic services.
  • You will work autonomously within your scope of practice, undertaking comprehensive patient assessments, planning and evaluating care, and supporting patients throughout their cardiac pathway.
  • You will coordinate and manage consultant and specialist nurse-led clinics, perform and interpret a range of cardiac investigations including ECGs, ambulatory blood pressure and Holter monitoring, and ensure patients receive timely, evidence-based care.
  • You will work closely with the multidisciplinary team to provide seamless care across community sites.
  • The successful candidate will act as a clinical role model, supporting and developing junior staff, promoting best practice and contributing to service improvement, audit and quality initiatives.
  • You will maintain accurate clinical documentation, identify and escalate clinical concerns appropriately, and contribute to maintaining a safe and effective clinical environment.
  • This role is primarily based at a designated community clinic, with flexibility to provide cross-site cover during periods of annual leave, sickness and service demand, ensuring continuity of care across the Community Cardiology Service.
