North Liverpool is the largest PCN in Liverpool with 21 practices and a weighted population size of approximately 150,000 patients.
Main duties of the jobResponsibilities- Conduct comprehensive medication reviews and assessments to identify and resolve medication-related problems for patients.
- Collaborate with physicians and other healthcare professionals to develop and implement personalised medication therapy plans.
- Provide patient education on medication use, potential side effects, and drug interactions.
- Monitor and adjust medication therapy based on patient response and laboratory values.
- Conduct medication reconciliation and identify discrepancies to ensure accurate and up-to-date medication lists.
- Participate in multidisciplinary meetings to discuss patient care and optimise treatment outcomes.
- Provide education sessions for GPs, practice staff, other health professionals and patient groups.
- Stay up to date with current literature and guidelines to provide evidence-based pharmaceutical care.

Person SpecificationQualificationsEssential- Master of Pharmacy degree from an accredited institution.
- Active pharmacy registration with GPhC
- Good knowledge of pharmacology, therapeutics, and medication management.
- Excellent communication and interpersonal skills.
- Ability to work well in a fast-paced environment and handle multiple priorities.
- Non-medical Prescribing qualification desirable or an interest in working towards obtaining the qualification
