Job summary
The Children and Young People's Continuing Care (CYPCC) service leads on the assessment, case management and oversight of the NHS National Framework for Children and Young People's Continuing Care across Kent and Medway. The service assesses, develops and oversees personalised care packages for children and young people with complex health needs in the community, working in partnership with social care, education and wider system partners as part of a multidisciplinary network

Main duties of the job
  • Providing clinical leadership by leading MDTs, training and mentoring the team and wider community partners.
  • Applying expert knowledge of the National Framework by leading assessment, implementation and review across Kent and Medway.
  • Working collaboratively with the Local Authority, contributing to joint assessments and resolving disputes.
  • Supporting the Local Resolution Process in participating in appeals and decision reviews.
  • Ensuring safe, effective and value for money care packages -- identifying cost improvement opportunities where appropriate.
  • Using advanced clinical expertise -- supporting the team with complex decision making.
  • Leading personalised care package development including through Personal Health Budgets.
  • Delivering CYPCC training and development and supporting workforce development across Kent and Medway.
  • Overseeing team performance and caseload management and being accountable for the clinical decisions and recommendations you make.

Person Specification
Qualifications, training and experience
Essential
  • You'll need to have a Level 6 Degree or equivalent qualification in relevant subject i.e. Nursing, Physiotherapy, Occupational Therapy or Speech and Language Therapy.
  • Proven ability to work effectively within your area of practice, supported by relevant experience. As well as managing complex caseloads and supporting/ mentoring others
  • You will be a registered health care professional with the Nursing and Midwifery Council or Health and Care Professionals

Knowledge and skills
Essential
  • You will need to be able to be able to use IT systems and databases
  • You will have knowledge of up to date clinical practice by using up- to- date skills and adhering to evidence-based procedures and clinical effectiveness frameworks. You will fully understand the role and remit of a health care worker.

Desirable
  • Having an awareness of developments within the NHS would be an advantage as well as an understanding of National Service Frameworks and NICE guidance.
  • You will understand where to signpost clients and families to alternative services to support behaviour change and improve health.

Working environment
Essential
  • A full, valid UK driving licence or a full driving licence issued by an EU country (not exchanged from a non-EU country) and the ability to travel independently are essential for this role.
