Job summaryA six-month secondment opportunity has arisen to provide additional specialist nursing capacity during the winter period across the COPD, Oxygen and pneumonia pathways.
The candidate will be assisting the three nurse specialist teams to support admission avoidance, early supported discharge and safe discharge from the hospital through the utilisation of the virtual ward pathways. The candidate will need to be confident with autonomous working and making independent decisions for the patient's pathway. The candidate must have appropriate knowledge and skills to be able to perform the role independently within weeks of commencing the position.
The candidate will need to be currently working on a respiratory ward and hold a full driving license to be able to perform community visits for the virtual wards or outreach visits for the home oxygen and ROCKET team.
Until 31 March 2027Main duties of the job- Responsible for identification of suitable patients to be transferred to the community acquired pneumonia (CAP) virtual ward and their ongoing care on the CAP virtual ward.
- Responsible for identification of suitable patients to be transferred to the COPD virtual ward and timely handover to the COPD VW nurses.
- Responsible for identification of suitable patients to be assessed by the 02 VW patients for weaning of Oxygen at home.
- Act as a clinical expert and professional resource to the patients, carers and muti-professional team.
- Directly or indirectly influence all aspects of care and management for patients entering any of the respiratory virtual wards and the long term condition teams ROCKET and HOSAR
- Provide clinical expertise, specialist advice and support across the whole spectrum of the service.
- Demonstrate in-depth knowledge and skills in patients that have CAP/COPD and require Oxygen therapy . Ensuring the delivery of appropriate evidence-based care.

Person SpecificationEducations, Training and QualificationsEssential- Educated to level 6 or equivalent level of education in nursing
- To have 2 years' experience working on a respiratory ward
Desirable- Independent/non-medical prescribing qualification
- Respiratory level 6 qualification
Knowledge and ExperienceEssential- Experience working autonomously in the assessment and management of patients with CAP/COPD and oxygen therapy
- Demonstratable knowledge and experience of COPD, Oxygen therapy and pneumonia patient assessments
- Evidence of effective multidisciplinary working
Desirable- Experience of working in the community
SkillsEssential- Ability to interpret relevant clinical investigations, including blood gases, Xrays , spirometry and blood results
- Advanced clinical assessment skills in pneumonia/COPD and oxygen therapy patients.
- Ability to perform Venepuncture and capillary blood gases
- Ability to communicate effectively with MDT/Patients /Carers and the wider respiratory team
