Job summary
Lambton Road Medical Practice is delighted to be recruiting a dynamic and patient-focused Independent Prescribing Pharmacist to join our friendly and supportive Primary Care Network (PCN) team.

This is an exciting opportunity for an enthusiastic pharmacist who is passionate about delivering high-quality patient care and making a real difference within the local community. Working closely with our GPs, nursing team, and wider multidisciplinary colleagues, you will play a key role in supporting patients with their medicines, managing long-term conditions, and promoting safe, effective prescribing.

Main duties of the job
The successful candidate will provide expert pharmaceutical care, supporting patients across a range of clinical settings. Main duties of the role will include:
  • Conducting patient-centred structured medication reviews to optimise medicines use and improve health outcomes.
  • Providing higher-level clinical administration and medicines management support to ensure safe and effective prescribing practices.
  • Managing and supporting the ongoing care of patients with long-term conditions, using your independent prescribing qualification where appropriate.
  • Identifying and addressing medicines-related issues, improving patient safety and reducing unnecessary hospital admissions.
  • Working collaboratively as part of a multidisciplinary team (MDT), contributing your specialist knowledge to support holistic patient care.
  • Supporting the delivery of national and local clinical priorities, including prescribing quality initiatives and service improvement projects.
  • Building strong relationships with patients, carers, and healthcare professionals to promote shared decision-making and person-centred care.

Lambton Road Medical Practice forms part of West Merton PCN, serving our local population of 35,000+. We are a forward thinking Practice and have a well established in house pharmacy team. We are located within 2 minutes of Raynes Park Train station, a bus ride to Wimbledon.

Job responsibilities
Main Duties of the Job
In this role, you will:
  • Support the Partners in implementing effective medicine management and patient care within the practice/s.
  • Identify areas for improvement and managing change
  • Deliver patient-facing clinical care: Conduct consultations, prescribe medication, run clinics (especially for long-term conditions), and support vulnerable or complex patients, including home visits.
  • Enhance patient understanding and outcomes: Educate patients and carers on medication use, improve adherence, and provide ongoing support to build confidence in treatments.
  • Ensure safety and quality of prescribing: Monitor high-risk drugs, handle medication incidents, carry out audits, and align practice with clinical standards (e.g., NICE) to improve safety and performance.
  • Collaborate, train, and develop services: Work within the multidisciplinary team, support staff training, liaise with external stakeholders (e.g., community pharmacists), and contribute to service improvement and professional development.
  • Improve medicines management systems: Oversee prescribing processes, medication reviews, repeat prescriptions, and ensure safe, effective, and high-quality use of medicines across the practice

Person Specification
Qualifications
Essential
  • An undergraduate Masters Degree or equivalent in Pharmacy.
  • Membership as practising pharmacist with The General Pharmaceutical Council.
  • Pharmacist independent prescriber registered with The General Pharmaceutical Council (Beneficial).
  • Relevant post registration experience of community, hospital and / or primary care pharmacy practice.
  • Commitment towards continuing professional development in accordance with requirements of The General Pharmaceutical Council.

Desirable
  • Post-graduate qualification to Diploma or Masters level in a recognised area of pharmacy practice or equivalent.
  • A working understanding of NHS policies, processes, structures, systems and clinical practices.
  • Knowledge of the Community Pharmacy Contract.
