Description

Job Summary
An exciting opportunity has arisen for a motivated and enthusiastic pharmacist with a specialist interest in mental health to join our highly supportive pharmacy team. We are seeking a compassionate and driven individual to work as a key member of the multidisciplinary team. The post will involve close collaboration with other community mental health services, urgent care teams, inpatient services, and primary care.

This is an excellent opportunity for a pharmacist committed to improving patient care within a supportive multidisciplinary environment. The role requires a well organised and flexible individual with strong communication and interpersonal skills, and the ability to work both independently and collaboratively.

As an integral member of the clinical team, you will address the pharmaceutical care needs of community-based service users and ensure safe and effective medicines management. You will work closely with clinical colleagues to support high-quality patient care, including compliance with regulatory standards and implementation of medicines safety alerts.

The post will provide opportunities for the following:
  • Provision of specialist pharmaceutical advice to the multidisciplinary team
  • Pharmaceutical care planning and clinical review of prescriptions
  • Involvement in multidisciplinary team meetings
  • Provision of medicines information to service users and staff

Main Duties of the Job
The post may also involve close liaison with other mental health services within the locality including; the urgent care pathway, primary care and in-patient mental health services.

You will support clinicians within the service with medicines management-related queries and improve the care of people with mental health issues through medicines optimisation.

Provide medicine-related advice and guidance to colleagues within the team and medicines optimisation support to clinicians within the service.

Support service users to be involved in decisions about their medicines to optimise treatment, support adherence, and improve outcomes

Deliver medicine optimisation interventions, including undertaking a Non-Medical prescribing role where appropriate.

Provide highly specialised advice and information on medicines to a variety of other services/agencies/organisations and carers both within and outside mental health services.

Attend strategic meetings e.g. locality governance meetings

Participate in and contribute to local and National strategies to improve services.
