Supporting a Home First approach is at the heart of our service. While many patients can return home directly following a hospital admission, some require a short-term stay in a rehabilitation setting providing 24-hour support.
Our Recovery Hubs play a vital role in facilitating timely hospital discharge and preventing avoidable admissions.
At Wharfedale Recovery Hub, you will provide rehabilitation to individuals with complex needs, supporting them to regain independence, maximise their functional abilities, and achieve their personal goals.
You will work as part of a welcoming multidisciplinary team committed to delivering safe, effective, and compassionate care.
We are dedicated to supporting your professional development through structured training, regular clinical supervision, and access to a strong peer support network.
This role forms part of a 7-day service, including weekends and bank holidays on a rotational basis. There may also be occasions when you are required to work across our other Recovery Hub sites.
Therefore, applicants must be able to travel independently across the locality and have access to a suitable vehicle for business use.
Main duties of the jobThis is an excellent opportunity to develop your rehabilitation skills while making a meaningful difference to patient outcomes and independence.
As a Senior Physiotherapist within our Short-Term Care Beds service, you will deliver high-quality physiotherapy assessment, treatment, and rehabilitation for patients requiring short-term inpatient rehabilitation.
The service operates between 08:00 and 18:00, seven days a week, and you will participate in a rotational shift pattern.
Working collaboratively with patients, families, and colleagues, you will assess individual needs and provide evidence-based physiotherapy interventions, advice, and education to support recovery, maximise independence, and promote self-management.
You will be an integral member of a multidisciplinary team, working closely with nursing staff, occupational therapists, therapy assistant practitioners, and other healthcare professionals to deliver coordinated, person-centred care.
You will contribute to the assessment, planning, implementation, and evaluation of rehabilitation programmes, ensuring care is tailored to each individual's goals and needs.
A key aspect of the role is empowering patients to achieve their rehabilitation goals and supporting safe, timely discharge planning.
You will also provide guidance and support to patients, carers, and colleagues, while contributing to service development and continuous quality improvement within the Recovery Hub.
The role is Fixed-Term until 31st July 2027.
