Main duties of the jobAs a Specialist Clinical Pharmacist, you will provide a high-quality, patient-centred clinical pharmacy service across the Trust. Responsibilities include delivering ward-based clinical pharmacy services, optimising medicines use, supporting safe and timely discharge through patient counselling and education, promoting adherence, and ensuring effective communication with community healthcare providers. You will also contribute to wider clinical pharmacy and departmental activities to support the Trust's pharmacy service objectives.
